Job summary

We are seeking an enthusiastic Digital and Transformation Lead to join our established team and be a part of a PCN that really makes a difference.

Supporting the PCN Clinical Director, Deputy Clinical Director and working with the PCN Strategic Manager you will oversee the management of the PCN team, manage the operational performance of PCN requirements and support PCN projects.

The role is for 30 hours per week and the pay banding listed is pro-rata.

Main duties of the job

The following are the core responsibilities of the Digital and Transformation Lead. There may be on occasion a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

The Digital and Transformation Lead is primarily responsible for:

  • Recruitment, retention and line management of all ARRS staff.
  • Development of training including specific and mandatory training for the ARRS team.
  • Provide HR support to the PCN staff including appraisals and any personal development plans.
  • Operational Management of the PCN including ensuring the DES, IIF and CAIP are met.
  • Undertake a lead role for the PCN GDPR compliance.
  • Provide monthly PCN performance data for a range of PCN activities.
  • Be the first port of call for all practices for PCN matters.
  • Attend operational meetings on behalf of the PCN.

In addition to the primary responsibilities, the Digital and Transformation Lead may be requested to:

a.Work in partnership with other local health and care organisations within the area on joint initiatives and service improvement projects

b.Act as the primary point of contact for NHS(E), ICB, community services and other external stakeholders

c.Be involvedin appropriate and agreed initiatives

d.Be involved in appropriate and agreed constituent practices improvement projects

Operational and Organisational:

Work in partnership with the PCN Clinical Director and Strategic Manager to ensure the development and smooth running of the Network

Work collaboratively with The Maltings Surgery Partnership, Summerfield Health Partnership and PCN Stakeholders to proactively contribute to strategic planning, financial management and development of the PCN and their constituent practices so that they are resilient for the future.

Establish optimised use of PCN teams, ensuring the PCN has appropriate building and internal accommodation to work effectively & efficiently; and proactively manage development and expansion plans for the business

Lead and provide guidance, with appropriate liaison with the operational managers of both Partnerships, on human resources, premises, and facilities management, liaising with practice team leaders to always ensure the smooth running of both the PCN and the constituent practices.

Develop and maintain effective communication within both practices, within the PCN, and with other relevant outside agencies such as neighbouring PCNs, the ICB, the Health Care Partnership and Integrated Care System partners, including but not limited to those within the wider NHS, social care and the voluntary sector.

Project manage any projects relating to the delivery of the PCN strategy, including finance, service redesign and innovations, Population Health Management and IT implementations.

Work with the PCN Clinical Director and Strategic Manager on strategic and operational planning processes to support the PCNs objectives

Co-ordinate the delivery of enhanced services and other service submissions on behalf of the Network.

Document and manage PCN workflow processes as they develop.

Human Resources:

Support the Clinical Director to provide leadership and management for workforce development, through assessment of clinical skill-mix and development of the network workforce strategy

Support the recruitment and retention of all PCN employed staff by working with the nominated recruiting Practice.

Responsible for the recruitment process, advertising vacancies, interviewing and putting forward a recommendation for employment to the PCN. Ensuring all employment checks are carried out prior to commencing employment and that induction and training programmes are in place

Direct line management for all PCN employed staff, undertaking reviews/appraisals/setting targets etc. In the short to medium term this role would require you to be first point of contact for all PCN employed staff, whilst working closely with lead clinicians in member practices.

Co-ordinate training and development of clinical/administrative staff where related to PCN activities; including organising educational events

Finance and Business Management:

Working with the finance lead, to be responsible for the oversight of the Networks financial systems and payments for the Network.

To work alongside the operational and business managers of both practices and the management teams of the Abbey Health PCN stakeholders within the Network to set up new financial systems and process as required by the PCN.

To monitor and check the accuracy of the any submissions for Enhanced Services and contractual requirements on behalf of the Network and ensure that all claims are submitted on a regular basis.

Ensure that the PCNs physical and financial resources are managed and maintained for maximum effectiveness through:

o Ensuring financial risks are recognised and appropriate action taken.

o Monitoring expenditure and identifying significant deviations from plan.

o Planning and monitoring income and cash flow to ensure that income is maximised and that funds are available as required.

o Ensuring all claims are submitted in good time and payment made and received.

o Ensuring the allocation of PCN resources is fair and equitable.

o Present financial plans (budgets, forecasts and what-if analysis), monitoring information and reports from management accounts and governance data against agreed metrics to the PCN Joint Management Board at an agreed regular interval.

o Liaise with an external accountant in preparation for end of year accounts

Ensure the PCN has a Business Continuity policy that is updated and in which everyone is kept fully aware
