Job summary
A fantastic opportunity has arisen for an experienced
Clinical Pharmacist to join our growing PCN team in a senior role. The role is primarily
based in the heart of St Albans City Centre, recently rated as one of the best
places to live and work in the UK by The Sunday times. You will be an integral
part of the primary care team, working alongside our GPs, Clinical Director, PCN
Manager and Practice teams to provide specialist pharmaceutical support and
safe, effective prescribing.
Main duties of the job
The post holder will work within their clinical
competencies as part of a multi-disciplinary team providing expertise in
clinical medicines management, provide face to face structured medication
reviews, manage long term conditions, management of medicines on transfer of
care and systems for safer prescribing, manage repeat prescription
authorisations and reauthorisation, acute prescription request, while
addressing both the public health and social care needs of patients in the GP
practices that make up the PCN.

our various providers and stakeholder to ensure we have a cohesive systems and
processes across the network. The post holder will provide services in line
with the fulfilment of the PCN DES e.g., SMRs as well as any local medicine
incentive schemes.
The post holder is a pharmacist, who acts within their
professional boundaries, supporting and working alongside a team of pharmacists
in general practice. In this role they will be asked to supervise a junior
pharmacist.
The post holder will also support to general practice
staff with regards to prescription and medication queries. They will help
support the repeat prescription system, deal with acute prescription requests,
and medicines reconciliation on transfer of care and systems for safer
prescribing, providing expertise in clinical medicines advice while addressing
both public and social care needs of patient in the GP practices.
The post holder will work as part of a multi-disciplinary
team in a patient-facing role. The post holder will take responsibility for
areas of chronic disease management within the practice and undertake clinical
medication reviews to proactively manage patients with complex polypharmacy

- Minimum of two years experience as a Pharmacist, demonstrated within a practice portfolio.
- Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice.
Desirable
- Experience of working in primary care.
- Experience of working in GP practice.
Qualifications

- Mandatory registration with the General Pharmaceutical Council.
- Completed independent prescribing qualification.
- Completed CPPE course.
