developed a pioneering position, which combines the role of Primary Care
Network Manager with that of the Nurse Manager within one of the Networks member
surgeries.

ensuring change towards more integrated, needs-led community healthcare;

Main duties of the job
The
post holder is required to develop and lead the provision of consistently high
quality, effective and cost efficient services across the PCN; and will
clinically lead and be managerially responsible for ensuring the delivery of
safe and up-to-date nursing care to The Maltings Surgerys practice population
The post holder will form an integral
part of the PCN leadership team working alongside the Clinical Director, the
PCN board members and the multi-disciplinary team members of each member
practice.
The focus of the role will be
predominantly managerial but the post holder will
demonstrate at all times a comprehensive clinical knowledge base and promote
excellence in nursing practice in a dynamic healthcare environment. As an
active member of the team, the post holder is expected to provide direct
patient care when necessary, acting as an expert practitioner and a role model
to the whole team. It is essential that they can use initiative and work
without direct supervision

Lattimore & Village Surgery is located in London Colney,
one of the biggest villages in Britain with a population of nearly 10,000
residents. London Colney has a lively and cosmopolitan community and work has
recently commenced on a new building for the surgery that has outgrown its

Job Role and Purpose
The
post holder is required to develop and lead the provision of consistently high
quality, effective and cost efficient services across the PCN; and will
clinically lead and be managerially responsible for ensuring the delivery of
safe and up-to-date nursing care to The Maltings Surgerys practice population.
As
the team leader for the Maltings Surgerys nursing team, the post holder is
accountable for nursing service delivery and for promoting the practice teams
as positive places for staff experience and development.
As a member
of the Joint Management Board and operating within a robust governance
structure, the post holder will be responsible, on behalf of the two practices,
for the development and delivery of all projects and services committed to or
commissioned by the PCN, working towards the achievement of PCN aims and
objectives. They also provide oversight of the financial management, resourcing
and regulatory compliance of the work of the PCN.
The post holder will form an integral
part of the PCN leadership team working alongside the Clinical Director, the
PCN board members and the multi-disciplinary team members of each member
practice. They will also liaise with relevant stakeholders within the local
area.
As a point of expertise, the post
holder will coordinate the successful achievement of all PCN service delivery
requirements, translate national and local policy into meaningful collaborative
change, and support practices to achieve improved outcomes for the local
population. The successful candidate will have excellent organisational,
communication and influencing skills. They will be confident in the collection,
analysis, presentation and use of data and will be confident in basic business
and contractual administration.

demonstrate at all times a comprehensive clinical knowledge base and promote

critical thinking and skills in clinical decision-making. They will work
collaboratively with the general practice teams to meet the needs of patients,
supporting the delivery of policy and procedures, and providing leadership and
direction for the nursing team.
