Job summary
To
contribute to the successful delivery of the key outcomes of the, The Limes
Medical Centre plan by provision of a high-quality nursing service to a defined
practice population. This will include assessment, health promotion, chronic
disease management, health screening and treatment in line with the
professional competence of the post holder. To be involved with the practice delivery
of National Service Framework targets and local Health Improvement priorities.
Identification of nursing needs of the population. Responsibilities to maintaining and improving
QOF data in line with the business plan and The Limes Medical Centre objectives.
Main duties of the job
- To
undertake comprehensive health assessment of patients at the point of
registration:
- Chronic
disease management.
- Contraception.
- Sexual
Health.
- Cervical
screening.
- Travel
health advice and immunisation following Patient Group Directives (PGD).
- Well
person / elderly health review.
- Venepuncture
and basic rule management.
- To
ensure that all nursing practice is evidence based applying current research.
- Childhood Immunisations
About us
At The Limes Medical Centre, we are proud of our dedicated and
experienced team. Our surgery is led by three GP clinical Partners who work
together to ensure the highest standard of care for our patients. In
addition to our three clinical partners, we have a Nurse Practitioner, Senior
Practice Nurse, HCA and Phlebotomist.
The day-to-day operations of the surgery are managed by our
Practice Manager and Deputy Practice Manager who ensures a friendly, welcoming
and professional atmosphere for both staff and our patients.
Supporting our operations,
we have a team of 8 skilled administrative and reception staff members. Their
training, efficiency, and commitment are vital to the seamless running of our
practice.

RESPONSIBLITIES/COMPETENCIES (including but not limited to):
1 Responsibilities
for Patient Care
- To
uphold the values of The Limes Medical Centre
- Provide
excellent patient clinical and customer care.
- Provide
a high standard of nursing care and a comprehensive screening and health
promotion service.
- To
develop, implement and audit chronic disease (e.g. hypertension, CHD)
management according to the health needs of the practice population. Maintain
accurate information systems, both computerized and manual.

continued clinical support as appropriate.
- Be able
to assess specific diseases in accordance with agreed medical/nursing
protocols.
- Undertake diagnostic health screening, health surveillance and therapeutic
interventions within a broader health promotion/public health strategy.

- To be
accountable for the nursing services and resources.

- Responsible for ensuring suitability and cleanliness of equipment used for
patient care, and that stock levels are appropriate, lead in infection control. 4 Communication
and Leadership

and Organising
- To be
involved in delivery of Salford Standards and QOF and Direct Enhanced services.

- Involvement in identifying and/or supporting practice development plans.
- Understand and apply legal issues that support the identification of vulnerable

in close collaboration with General Practitioners, other Primary Care Nurses,
Allied Professionals, Social Services, voluntary sector, the ICB and other
appropriate agencies to ensure continuity of patient care.

- Minimum12 months experience of working in General Practice
- NMC registered
- Completed Cytology training
