Job summary

The main responsibilities of the role include:

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Running asthma, COPD and diabetes clinics and reviews.
  • Experience with hypertension care, leg ulcer level 1 training and menopause training are all preferable.

Main duties of the job

Portmill Surgery has a fantastic opportunity for an experienced practice nurse, or qualified registered nurse to join our team on a part-time basis, for 16-37 hours per week. Hours and days of work can be negotiated.

All applicants should have the following qualifications, registrations, skills and experience:

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications; wewould be willing to consider a candidate with suitable experience for the role, whilst they do the FoPN course alongside.
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates or willingness to train in these areas.
  • Evidence of clinical skills and experience in treatment room consultation, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.

About us

Portmill Sugery is a friendly, dynamic, well-established partnership of 7 doctors who enjoy working with a salaried GP, 6 nurses, a Health Care Assistant, PCN additional roles and a team of highly trained and motivated staff. We are a training practice and an active and enthusiastic member of the Hitchin and Whitwell PCN.

We provide general medical services to c14,000 patients in a desirable commuter belt town and are ideally looking for an experienced nurse to become part of our team.

As well as general practice, we:

  • offer extended access hours appointments to enable all of our patients to access medical care at evening and weekends.
  • work collaboratively with the Primary Care Network to provide additional services, social prescribing as well as both adult and young people coaching services.
