Job summary

BRADLEY'S PRACTICE - This post is based at both sites, Mold & Buckley. We are a Vision practice with two newly built buildings which service 15000 patients. We are a dispensing and training practice. The role of a Practice Nurse would be to provide nursing services to patients of the practice including assessment, advice within the professional competence of the post holder, screening and treatment. The post holder will work in accordance with the NMC code of conduct.

Main duties of the job

We are looking for an experienced Practice Nurse who can provide telephone/ face to face consultations for any nursing treatments to patients in participation with general practitioners or independently to agreed protocols and pgds.

Recording clear consultation notes on the Vision consultation system.

About us

Here at Bradleys we are a 2 sited practice with a full compliment of clinical and non clinical staff. They include:- 10 GP partners, 2 ANP's, 1 Pharmacist, 1 Associated physiotherapist and Audiologist, 4 practice nurses and 3 HCA's.

This team is backed up with 2 managers , Clinical Governance Officer and other supervisory staff in addition to reception and admin staff.

Main purpose of post:-

To provide nursing services to patients of the practice including assessment, advice within the professional competence of the post holder, screening and treatment. The post holder will work in accordance with the NMC code of conduct.

1 TO PROVIDE ASSESSMENT, SCREENING, TREATMENT SERVICES AND HEALTH EDUCATION ADVICE

  1. Provides nursing treatments to patients in participation with general practitioners or independently to agreed protocols and pgds. To also assist in the treatment of iucd insertions and removals and the administration of gonderalins.
  2. Provides general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary e.g. Well woman, hypertensive, diabetic, asthmatic clinics, chd clinics, weight management, nursing home assessments, heart failure monitoring, impaired glucose tolerance monitoring, smoking cessation advice and treatment, provision of level 4 INR service and chronic kidney disease.
  3. Advises patients in general health care and minor ailments with referral to general practitioners as necessary.
  4. Complete re-auths on a daily basis as required.
  5. Administer and offer advice with regards to travel vaccinations and immunizations for adults and children.
  6. Carry out blood taking.
  7. Operation of nurse triaging system including passing on referrals to appropriate members of the primary health care team and liaising with LHB.

2 ADMINISTRATIVE AND PROFESSIONAL RESPONSIBILITES

Participates in the administrative and professional responsibilities of the practice team especially in relation to the new GMS Contract April 2004 which works within the specification of QOF and protocols where available/enhanced services.

  1. Ensures accurate notes of all consultations and treatments are recorded in the patients computerized notes and that recalls for monitoring are adhered to, Also to provide high quality summarized data in relation to awarding priority codes.
  2. Ensures accurate completion of all necessary documentation associated with patient health care.
  3. Ensures collection and maintenance of statistical information required for regular and ad-hoc reports.
  4. Attends and participates in all practice and practice nurse meetings as required.
  5. Assists in the formulation of practice philosophy, strategy and policy.
  6. To assist with prescription requests and to help with doctors administrative queries.

3 TREATMENT ROOM SUPPLIES AND EQUIPMENT

  1. In participation with the practice manager, ensures the maintenance of equipment and stock levels relating to patient care and undertakes necessary training in latest technology.

4 OPPORTINISTIC SCREENING

  1. Provides opportunistic screening e.g., immunizations including tetanus, typhoid, Flus etc.

5 PATHOLOGICAL SPECIMENS AND INVESTIGATORY PROCEDURES

  1. Undertakes the collection of pathology specimens including intravenous blood samples, swabs, cervical smears etc.
  2. Performs any investigatory procedures requested by the general practitioners e.g. MSU, Dipstick urines and undergoes checks with patients who use nebulisers.
  3. Carry out ecg, Spiro testing when required.

6 RESEARCH PROJECTS

  1. Co-operates and participates as required in any research projects within the practice.

7 EDUCATION AND TRAINING OF STUDENTS AND PRACTICE STAFF

  1. Participates in education, training and the provision of mentorship to students in all disciplines and provides an induction for practice staff and in particular nursing staff.
  2. Supervise Health Care Assistants in clinical and administrative responsibilities.

8 LIAISON WORK

  1. Liaises with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard for confidentiality.

9 PRACTICE DISPENSARY

  1. Provides medical advice where necessary and initiates medication to dispensed patients with the approval of the GP.
