Job responsibilities

Practice Nurse

Main purpose of post:-

To provide nursing services to patients of the practice including assessment, advice within the professional competence of the post holder, screening, and treatment. The post holder will work in accordance with the NMC code of conduct.

1 TO PROVIDE ASSESSMENT, SCREENING, TREATMENT SERVICES AND HEALTH EDUCATION ADVICE

a) Provides nursing treatments to patients in participation with General Practitioners or independently to agreed protocols and PGDs.

b) Provides general and specific health screenings to the practice patients, within agreed protocols. These may include Hypertensive, Diabetic, Asthma, CHD, Lifestyle Management. Heart Failure monitoring, CKD and specialist clinics.

c) Recognising when to refer to GPs or specialist HCP.

d) Administer and offer advice regarding both childhood and adult immunisations.

e) To provide comfortable and discreet cervical screening services to patients

f) Phlebotomy when required.

2 ADMINISTRATIVE AND PROFESSIONAL RESPONSIBILITES

Participates in the administrative and professional responsibilities of the practice team especially in relation to the new Unified GMS Contract 2023 Wales which works within the specification of QAIF and protocols where available/enhanced services.

a) Ensures accurate notes of all consultations and treatments are recorded in the patients computerized notes and that recalls for monitoring are adhered to, also to provide high quality summarized data in relation to awarding priority codes.

b) Ensures accurate completion of all necessary documentation associated with patient health care.

c) Ensures collection and maintenance of statistical information required for purpose of audits and progress reports.

d) Attends and participates in all practice and practice nurse meetings as required. Contributes towards progression of nursing team.

3 TREATMENT ROOM SUPPLIES AND EQUIPMENT

a) Contributes to the maintenance of equipment and stock levels relating to patient care and undertakes necessary training in latest technology.

4. PATHOLOGICAL SPECIMENS AND INVESTIGATORY PROCEDURES

a) Undertakes the collection of pathology specimens including intravenous blood samples, swabs, cervical smears etc.

b) Performs any investigatory procedures as required.

6 RESEARCH PROJECTS

a) Co-operates and participates as required in any research projects within the practice with the aim of personal and professional development.

7 EDUCATION AND TRAINING OF STUDENTS AND PRACTICE STAFF

a) Participates in education, training, and the provision of mentorship to students in all disciplines and provides an induction for practice staff and in particular nursing staff where appropriate.

b) Supervise Health Care Assistants in clinical and administrative responsibilities when required.

8 LIAISON WORK

a) Liaises with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard for confidentiality.

9 PRACTICE DISPENSARY

a) Provides medical advice where necessary and initiates medication to dispensed patients with the approval of the GP.

10 PROFESSIONAL DEVELOPMENT

a) Attends courses and study days in line with professional development needs.

b) Agrees to appraisal process to identify and agree on desired career goals and developmental achievements.
