Job summary

We are looking for an experienced Practice Nurse to join our friendly practice nursing team. Chronic disease qualifications are essential in either Asthma or Diabetes. Qualifications in both are an advantage!

This is a part time role for 22.5 hours per week. Flexible hours available.

Main duties of the job

We are looking to welcome an experienced, qualified general practice nurse into our practice to assess, implement, and evaluate treatment programmes for our patients. Monday to Friday cover is required, with hours to be agreed. There is scope for flexibility. The post-holder will work as part of the medical centre team providing immunisations, wound care, screening and health education to patients registered with the practice. They will use their experience, knowledge, skills and qualifications to support effective management of patients with COPD, asthma, diabetes, tissue viability and other chronic long term conditions.

About us

We are a busy, friendly and supportive practice catering for 11,000 patients. We are located in Cherry Hinton (Fishers Lane) with a branch two miles away, towards town on Seymour Street. This role will require you to work at both sites.

The clinical team is made of up Partner and Salaried GPs, a clinical pharmacist, one full time and one part-time practice nurse, one of whom is the team leader. We also have two full time HCAs. We are also supported by a Primary Care Network team.

Job responsibilities

SUMMARY OF MAIN DUTIES

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.

The surgery operates at two locations and this post is required to work at both. In addition to this the practice has a number of patients who are housebound or live in care homes and you may be required to visit these patients in the community.

Professional

Ensure clinical practice is safe and effective and remains within the boundaries of competence and acknowledge professional limitations; ensuring in-house onward referral or GP advice is obtained as needed.

Adhere to all practice and professional policies and procedures.

Clinical

While you are required to undertake all standard treatment room nursing activities, such as dressings, venepuncture and physiological measurements etc, these should be devolved to the HCA team where time, skill set and patient convenience allow.

Carry out chronic disease clinics pertinent to your skill set and as agreed by the Partners/Practice Manager.

Ensure an overview of the practices QoF achievement is maintained and related clinical activities are carried out in a timely manner.

Provide holistic health assessment and health promotion service to patients, including, alcohol intake, weight management, smoking and diet.

Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including, but not limited to, vaccinations and medicines, safe sex, food hygiene, sun protection

Intramuscular injection and sub-cutaneous injection of drugs

Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions or patient specific directions.

Perform a holistic assessment and service provision of patients attending for ear irrigation.

Perform venepuncture according to local guidelines

Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines and local formularies.

Take ECGs

Able to recognize and manage anaphylaxis according to current UK guidelines.

Able to perform Cardio-Pulmonary Resuscitation according to current UK guidelines.

Administration/Other

Ability to obtain and document informed consent (either verbal or written) and offer and record chaperoning.

Ensure computerised patient notes are accurate, timely and appropriately read-coded.

In conjunction with your nursing colleagues ensure clinical equipment, consulting rooms and stock levels are maintained for the practice.

Undertake administrative duties consistent with the role.

Undertake ad hoc duties as required within the scope of the role and experience.

  • Discuss with other members of the team how the policies, standards and guidelines will affect own work and liaise with Lead Nurse/Practice Management to suggest service improvements
  • Participate in audit as required.

Educational

Identify personal development and training needs in conjunction with the Senior Practice Nurse & Practice Manager

Participate in continuing education and maintain a contemporary level of professional knowledge and skills and ensure this is recorded in the practice training logs.

You may be required to attend training events outside your normal working times/days in agreement with the Lead Nurse/Practice management.

Contribute to colleague learning by sharing education and feeding back to the team via appropriate methods e.g. nurse meeting, email etc.
