Job summary

We are looking to recruit a clinical pharmacist to develop medicines optimisation services within our Wolverhampton North Network (WNN).

The succesful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promoting self care.

The candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.

Main duties of the job

The successful candidate will work as part of a multdisiciplinayr team to develop and run processes for repeat prescription re-authorisation, management of medicines on transfer of care and systems for safer prescrbing. You will also undertake structured medication reviews for patient in care homes and community.

Experience in General Practice and knowledge of EMIS and Docman 10 is advantegous.

About us

In return we will support to develop the role of the Clinical Pharmacist through a training pathway (currently the CPPE pathway) with support of Gereral Practitioner clinical supervisor and Senior Clinical Pharmacist.

Qualifications

Essential

  • Educated to equivalent level of experience of working at a senior level in specalist area

Desirable

  • Educated to Masters level

Experience

Essential

  • Knowledge of medicines optimisation, acquired through post graduate diploma or equivalent experience or training
  • Evidence of post qualifying and continuing professional development
  • Member of General Pharmaceutical Council
  • Demonstrates understanding of legal, governance and ethical frameworks within which pharmacists and primary/secondary care organisations work

Desirable

  • GPhC Independent Prescribing Qualification
  • Experience of working in a GP practice / Primary Care Setting
  • Evidence of expereince in using Independent Prescribing
  • Experience of undertaking evidence reviews
  • Experience of clinical audit
  • Ability to manage staff especially for clinical supervision

  • Excellent clinical knowledge and ability to apply to practice
  • Current and valid UK driving licence with access to a car
  • Ability to critically analyse complex information and evidence and communicate the findings clearly in simple terms

  • Experience of a patient focused role and consultation skills to apply knowlege in a format patients/carers can
