Job summary
This is an evolving role split between dispensary duties and developing clinical pharmacy technician responsibilities.
To promote and facilitate safe and effective high quality and cost-effective prescribing and management of medication.
To undertake prescribing audits and work within Standard Operating Procedures (SOPs).
Support with the management of Medicines Information as part of a medicine's information service.
Process repeat prescriptions, and undertake Dispensary duties.
Complete the reconciliation of medicines detailed in patient medical documents.
Using prescribing data, information, knowledge and technology to contribute to the management of the prescribing budget and improve, inform and support a portfolio of Medicines Optimisation projects, services, and initiatives.
Main duties of the job
Main Duties of the Job:
To dispense medications as directed by the GP for Dispensing Patients and manage stock appropriately
Support the repeat prescription process, ensuring safe, accurate, and timely issuing of medication.
Undertake medicines reconciliation for new patients and following hospital discharge.
Assist with medication reviews under supervision of the Clinical Pharmacist or GP.
Provide patients with advice on the safe storage, use and disposal of medicines, within professional competence.
Contribute to delivery of QOF, IIF and Enhanced Services related to prescribing and medicines safety.
Implement and monitor prescribing safety alerts, audits and medicines optimisation initiatives.
Support, advise and train administrative staff on repeat prescribing protocols.

You will be working in a team of Practice Nurses and Health Care Assistant's. The Practice was last inspected by the CQC in August 2023 and rated as Outstanding. - To undertake relevant clinical audits to ensure patient safety is maintained and medicines optimised, and act upon the results liaising with patients and/or practices.
- To monitor national medical and patient safety alerts via Eclipse, to ensure actions are completed in a timely manner.
- To support practice in maintaining CQC standards for medication safety.
- To undertake thorough reasoned analysis on possible medications switches which maintain the quality of therapy of the original drug with a lower average unit cost.
- Inform patients regarding changes to their medicines in a professional manner and have the capacity to support and reassure patients when necessary, dealing with any issues or concerns that may arise within the boundaries of the post.
- To record all relevant pharmaceutical information in the patient's medical notes.
- Carry out housekeeping whilst in the patient record e.g. medicines synchronisation
- To review prescribing processes which maintain or increase patient safety but also eliminate waste and duplication.
