Main duties of the job
We are seeking a qualified nurse with experience in diabetes management to join our established and supportive nursing team. The successful applicant will primarily lead on diabetes care, with the opportunity to develop and undertake additional practice nursing duties. Full support and training will be provided for the right candidate to build these skills as part of the wider nursing team.
Ideally we are looking for 25-30hrs per week
over 3 days, but we are willing to consider options for the right person.
About us
We are a well-run, high achieving practice in
a pleasant part of Halifax nestling on the border of Huddersfield.
We cover a list size in excess of 11K patients
with 5 Partners GPs, 2 Salaried GPs, 3 ANPs, 2 Nurses, HCA and Phlebotomist. We
provide a range of in house enhanced services and work with nearby practices to
deliver additional appointments outside our core hours of 0800-1830 Mon-Fri.

Duties and Responsibilities
This role is primarily focused on the management of patients with diabetes within the practice. The postholder will be responsible for delivering high-quality diabetes care and working closely with GPs and the wider nursing team.
There will be the opportunity to carry out other practice nursing duties depending on experience and interest. Support and training will be provided where needed, but the main emphasis of the role will remain diabetes management.
Main Responsibilities Diabetes

Provide routine reviews and ongoing management for patients with Type 2 diabetes and pre-diabetes

Work in line with current guidelines and QOF requirements

Support patients with lifestyle advice and medication reviews
-
Monitor diabetes-related risk factors and complications
-
Review and act on blood results relevant to diabetes care

Provide education and support to help patients manage their condition
-
Maintain accurate clinical records and diabetes registers
Other Practice Nursing Duties (Desirable)Depending on skills and service need, the role may also include some of the following:

Cervical cytology
