Main duties of the job

We are looking for a qualified practice nurse with experience of chronic disease and sexual health management to join our established nursing team.

Ideally we are looking for 25-30hrs per week over 3 days, but we are willing to consider options for the right person.

About us

We are a well-run, high achieving practice in a pleasant part of Halifax nestling on the border of Huddersfield.

We cover a list size in excess of 11K patients with 5 Partners GPs, 2 Salaried GPs, 3 ANPs, 2 Nurses, HCA and Phlebotomist. We provide a range of in house enhanced services and work with nearby practices to deliver additional appointments outside our core hours of 0800-1830 Mon-Fri.

Duties and Responsibilities:

The duties and responsibilities to be undertaken will include most or all of the items in the following list; however duties may be varied from time to time under the direction of the GPs/Practice Manager, dependent on current and evolving practice workload and staffing levels.

PROFESSIONAL

  • Chronic disease management
  • Sexual health treatment/advice (including depo injections; coil checks)
  • Smear taking
  • Travel immunisations/advice
  • Child immunisations
  • Prostap/zoladex vaccinations
  • Flu vaccinations
  • B12 vaccinations
  • Spirometry
  • ECG recording
  • H-Pylori breath tests
  • Ear syringing
  • Patient height and weight (BMI)
  • Phlebotomy
  • BP monitoring
  • Patient health checks
  • Urinalysis
  • Wound care
  • Suture removal
  • Smoking cessation advice
  • Health/life style advice
  • Swab taking

ORGANISATIONAL

  • Advance preparation blood test request forms for own patients.
  • Maintain and order stocks of childhood vaccinations.
  • Maintain and order stock of treatment room equipment/supplies.
  • Maintain and order stocks of travel vaccinations
  • Clearing and stocking of own consulting room
  • Monitoring and stocking of allocated anaphylaxis pack.
  • Preparing environment and equipment before, during and after patient care interventions including own consulting room and other rooms when assisting with other procedures.
  • Maintain/update patient clinical records using practice templates to ensure all relevant information is captured accurately and coded appropriately.
  • In accordance with practice protocol ensure temperature of allocated medical fridges are checked and recorded daily, escalating any issues as appropriate to the Practice Manager.
  • In accordance with practice protocol and prevention of legionellas disease ensure water is run weekly through designated taps; documentation signed.
  • Ensure patient group directives are up to date and read/signed by all relevant personnel.
  • Undertake the collection, storage and despatch of pathological specimens including blood samples, swabs, smears etc
  • Knowledge of practice procedures and work in accordance with written protocols.

Help raise awareness of health and well-being

Assisting in the assessment and surveillance of patients health and well-being

  • Notifying the GP of any significant patient issues
  • Liaising with and assisting members of the administrative team
  • Be aware of and complete/make arrangements to complete any outstanding QOF tasks as per patients clinical records.
  • Attend and participate in practice meetings as requested.
