We are looking for a qualified advanced practitioner to join our established team. We cover a list size in excess of 11K patients with 6 GP Partners, 3 Salaried GPs, 2 ANPs, 3 Practice Nurses, a Healthcare Assistant, a Phlebotomist and admin staff. In addition to this we are also supported by a team of Primary Care Network clinicians including Clinical Pharmacists, First Contact Physios, Mental Health Practitioners, Social Prescriber Link Workers and Care-Co-ordinators. We provide a range of in house enhanced services and work with nearby practices to deliver additional appointments outside our core hours of 0800-1830 Mon-Fri.
We are a well-run, high achieving practice in a pleasant part of Halifax nestling on the border of Huddersfield.

Main duties of the job
The post holder will work as part of the primary healthcare team providing and maintaining a high standard of medical care for all patients of the practice.
The post-holder will have a nationally recognised prescribing qualification and will prescribe within their area of competence.
Working in accordance with a recognised Council Code of Conduct (eg NMC; GPhC etc) and with reference to practice agreed protocols and guidelines, the post-holder will provide assessment, treatment, preventative care, screening and health education to patients. This will be via a variety of methods including face-to-face, telephone, video and e-consultations.
Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with complex needs and/or minor illness; seeing and advising people in respect of their continuing medical needs.
Participate in the administrative and professional responsibilities of the Practice team.
Attend and participate in practice meetings as requested.
About us
We are a training practice with 3 GP trainers and currently 5 GP Registrars.
We are supporting the development of an ANP and Practice Nurse, participating in schemes available via the West Yorkshire Training Hub.
We work with both Leeds and Lancaster Universities offering several placements each academic year for trainee PhysicianAssociates and Yr3 & Yr5 Medical Students.
We are a Research Accredited practice participating in various studies throughout the year - often several projects simultaneously.
