Practice Nurse- Newly Qualified/New to Primary Care

Job summary

We are looking for a newly qualified or new to Primary Care Practice Nurse.

You will be employed as a Practice Nurse on a temporary contract for 14 Months. You will be working in our busy GP practice in the Castleford area with a list size of over 11,000 patients.

You will complete a 12 month training programme with West Yorkshire Primary Care Workforce & Training Hub on General Practice Nurse -VTS.

In the first 6 months you will complete several short courses to gain practical skills including: Phlebotomy, Vaccination & Immunisation, Wound Care, CVD, Mental Health, Asthma & COPD, Diabetes, Learning Disabilities, Motivational Interviewing, Quality Outcome Framework (QOF) and contraception & Sexual Health.

In the second six months you will complete a long-term condition module. Throughout the 12 months, there will be monthly peer group learning sessions where the whole group come together to learn and share through action learning sets.

Main duties of the job

You will help to deliver nursing services to our local population.

We are looking to recruit a newly qualified or new to primary care nurse. The course start date is 3rd November. The applicant must be able to start before this date.

Own transport is highly recommended for this post. Able to travel to work and attend all training courses provided.

Hours of work are 35 hours (7 hours per day, Monday- Friday) some flexibility is required to complete training days.

Our practice has a very strong training ethos. We train GP Registrars and medical students.

Job responsibilities

The practice nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of practice.

Job responsibilities:

Be part of the Practice Health Care team providing high quality and forward thinking nursing care to meet the holistic healthcare needs of the practice population. To work within NMC Code of Conduct and to agreed practice standards and protocols.

The Practice Nurse will have support from their workplace mentor to be accountable for their own professional actions.

After relevant training, undertakes procedures to include, phlebotomy, blood glucose testing, BP checks, urinalysis, peak flow, new patient health checks, pregnancy testing, stitch/clip removal.

Recognises and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.

Undertakes regular checking of all resuscitation equipment.

Works within all relevant practice policies and procedural guidelines e.g., infection control, chaperoning, Patient Group Directions (PGDs).

Enables, encourages, and supports individuals, families and groups to address issues which affect their health and social well-being.

Represents individuals and families interests when they are not able to do so themselves.

Develops and maintains the health promotion material within the practice.

Makes referrals to statutory and voluntary agencies as appropriate.

The Practice Nurse has a role in promoting good leadership for all grades of staff

Contributes to the development, monitoring and evaluation of care delivery by the Primary Health Care Team.

Promotes effective communication and relationships within the team.

Partakes in clinical supervision for own development.

Actively participates in clinical and practice meetings.

Develops own knowledge and practice to meet objectives/changes in service.

Contributes to practice policy development as required.

Monitors and uses resources appropriately within the team e.g., dressings, vaccines, stationery.

Makes effective use of the clinical system by ensuring the consistent and accurate entering of data and use of Read Codes.

Facilitates the use of up to date information technology in order to collate accurate and timely information as and when required.

Makes recommendations to raise standards of practice and service delivery.

Frequent exposure to body fluids, blood and wounds.

Occasional exposure to aggressive behaviour.

Potential requirement to travel between sites.
