Salford Medical Centre is a GP practice with over 13,000 patients. We also have a branch practice: The Quays Practice which currently operates out of Ordsall Health Centre (M5 3PH)
We are looking to welcome a new Advanced Nurse Practitioner to our team. We are open to part time or full time (3-5 days).
Previous or current experience working in General Practice is essential.
What the practice requires:
- You must be able to work between the hours of 8am - 6:30pm
- You must be able to work at BOTH sites.
- We may require flexibility to cover sickness / annual leave (sometimes at short notice)

Main duties of the job
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
About us
Salford Medical Centre is one of 10 practices that make up the Salford South East Primary Care Network.We are a long standing high achieving practice and at our last assessment were rated as Good by the CQC.
We work using the EMIS clinical system as well as using Footfall, Docman10, Vision 360, Ardens, Accurx, iPlato, & Outlook.
Our practice has seen a lot of change in the last 2 years due to the opening of our branch site. We have grown from being under 4000 patients to 12,000 patients. The patient demographic at each of our sites is very different which provides variety in speaking to people from all walks of life as well as a different working environment and with different members of the team.
Due to our practice expending our clinical & administration teams have grown and diversified. Our team is a mix of ages and backgrounds giving a wealth of knowledge & support for any new starters.
Our clinical team consists of: 1 GP Partner, 5 Salaried GP's, 2 locum GP's, 1 ANP, 1 Physician Associate, 2 Practice nurses, 2 HCA's, as well as access to our PCN Pharmacists & MSK Practitioners
Our clinical, nursing & administrative teams all work harmoniously together as they are all patient focused and driven towards providing the best care for our patients. We also continuously review our systems & processes in order to better meet our patients needs.

Job responsibilities
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Undertake the collection of pathological specimens
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Extended and supplementary prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF etc.), complying with local and regional guidance
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters, in accordance with local and national policies
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
