Job summary
We are seeking a highly motivated Practice Nurse to join our friendly and experienced team of nurses and health care assistants.
A team committed to delivering high standards of patient care.
You will need to have previous experience working in a primary care setting and have a fully rounded set of skills.
The role requires a well presented individual with a calm and methodical approach to work and the ability to interact empathetically and professionally with the public.Alongside the ability to communicate effectively with colleagues throughout the practice.
Main duties of the job
Management of chronic disease -monitoring and development of individual care plans, agreeing these as appropriate with the patient and other health professionals. A special interest in asthma and diabetes is preferred.
Family planning & Women's health
Travel ealth advice and vaccinations.
Child immunisations
QOF, DES & LES Implementation.

Situated in Kent, our practice currently serves 14,000 patients and delivers additional services such as minor surgery, travel vaccinations and family planning.
We are a training surgery and have a regular intake of both medical and nursing students. We encourage and support our employees with training and development and take pride in our professional and caring approach.
We are looking for a dedicated, passionate, skilled, and motivated individual to support our well established, friendly, and caring nursing team, which includes three registered nurses, two HCAs and a phlebotomist.

- Providing support to HCAs
- Conduct health assessments and develop individualized care plans.
- Provide patient education on health management and disease prevention.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Maintain accurate patient records and documentation.
- Mentoring students
- QOF, DES & LES Implementation.
Requirements:
- Registered Nurse with valid NMC registration.
- Strong knowledge of patient care practices and anatomy.
- Excellent communication and interpersonal skills.
- Ability to work effectively within a team and independently.
- Commitment to continuous professional development
Days and hours are negotiable but must include Monday & Friday.
