Job summary

Derwent Practice is a friendly, patient-focused Practice seeking an enthusiastic and motivated Practice Nurse to join our dedicated clinical team. This is an excellent opportunity for a qualified nurse with aminimum of one years experience in practice nursingwho is passionate about delivering high-quality, evidence-based care within a primary care setting.

This post is a permanent position at the Practice and we can be flexible for the right candidate on working days - please note Saturday working is expected, which is done on a rota basis in addition to the post holders weekly contracted hours.

Main duties of the job

We are seeking a skilled and compassionate Practice Nurse to deliver high-quality patient care within a busy primary care setting. The role includes undertaking diagnostic procedures such as venepuncture, ECGs, blood pressure monitoring and specimen collection, alongside managing chronic disease clinics and maintaining effective call and recall systems.

You will provide chronic and post-surgical wound care, administer immunisations and injections under PGDs, support national screening programmes, and assist with contraceptive and minor surgery services. The role also involves maintaining vaccine cold chain compliance and leading on infection prevention and control.

The successful candidate will be proactive in promoting healthy lifestyles and supporting patients in the management of their long-term conditions.

Clinical Responsibilities:

  • To be responsible for diagnostic procedures for example, venepuncture, blood pressure, height/weight, ECG and any specimens.
  • To advise patients on general health care and promotion of healthy lifestyles.
  • To manage patients with Chronic Disease, running specific clinics and maintaining call/recall systems.
  • To be responsible for chronic wound care as well as post-surgical wound care.
  • To administer injections under an individualised prescription or PGD within training limits.
  • To maintain the cold chain of vaccines. Including safe storage, rotation and disposal of vaccines and drugs.
  • To administer immunisations for example, influenza, pneumococcal, tetanus and childhood immunisations.
  • To participate in national screening work such as cervical cytology.
  • To provide directly and assist GPs with contraceptive services.
  • To assist GPs with minor surgery as required.
  • As part of a team, lead on infection prevention and control.

Core Duties:

  • To work independently and as part of a busy Nursing Department, to meet the needs of patients and service delivery.
  • To be an active and integral member of the Nursing Team, attending team meetings and training sessions, supporting colleagues and working flexibly to cover annual leave or sickness as well as enhanced hours including Saturdays on a rota basis.
  • To act as chaperone if required/requested by patients and colleagues.
  • To work alongside HCAs and Student Nurses to develop their skills and offer mentorship, guidance and support to improve standards of care if needed.
  • To assist in maintaining a safe environment for the nursing team, patients and co-workers, ensuring that all statutory and local regulations (e.g. Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed
  • To ensure data is recorded accurately and timely onto patient records using SystmOne, with due regard for record management and patient confidentiality.
  • To ensure data is accurately recorded and coded onto relevant templates and documentation associated with patient health care.
  • To maintain and stock examination rooms and treatment rooms with agreed consumables and clinical equipment / instruments.
  • To collect data and participate in regular Practice audits and research projects.
  • To be aware of quality standards and targets as set out in NICE guidelines, NHS contracts, Quality and Outcomes Framework, etc., working to achieve these standards at all times.
  • To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
  • To maintain annual registration with the NMC and other professional bodies (as required). To adhere to the Code of Professional Conduct.
