Derwent
Practice, on behalf of North Riding Community Health Network seek an enthusiastic,
forward thinking Pharmacy Technician to work alongside our Clinical Pharmacists
within our PCN.
Clinical
Pharmacy is an exciting programme of transformation to develop a new model of
care which integrates pharmaceutical skills into general practice, to optimise
patient care.
This
role will:
lead improvements to maximise safe, cost
effective best practice in prescribing,
support structured medication reviews
improve repeat prescribing processes,
including promotion of repeat dispensing and online ordering
minimising clinical risk and aiming to
reduce wasted medicines.
encourage the development of better
understanding of the principles of medicines optimisation throughout the
practice teams and promoting good practice in line with therapeutic
developments
assist the PCN in achieving national
requirements, NICE implementation and utilisation of medicines optimisation
initiatives
Main duties of the job
The successful candidate will be based in a local cluster
of General Practices as part of North Riding Community Health Network.
They
will be dedicated, reliable and person-focussed and enjoy working with a wide
range of people.
They will have good written and verbal communication skills
and strong organisational and time management skills.
They will be highly
motivated and proactive with a flexible attitude, keen to work and learn as
part of a team and committed to providing high quality care.

Our PCN is located on the edge of the North
York Moors National Park and our surrounding area includes the Howardian Hills
an area of outstanding national beauty, white sandy beaches of the Yorkshire
Coast and picturesque views of the Wolds not forgetting the market town of
Malton, Yorkshires famously renowned foodie destination.
This is a picturesque
tranquil area within easy reach of York and excellent road and rail links to
major cities. The area offers affordable housing and an abundance of outdoor
activities.
Our PCN has a combined list size of 39,000
patients, each member practice has a CQC rating of good in all areas and 3 out
of the 4 practices are dispensing practices.

Core Responsibilities:
- Support the role of PCN
Clinical Pharmacists in the delivery of clinical pharmacy services
- Perform medicines
improvement work to maximise safe, cost-effective best practice in
prescribing under the supervision of the GP or Clinical Pharmacist
- Support the Clinical
Pharmacists in Structured Medication Reviews (e.g. organise necessary
monitoring tests before SMR)
- Assist Clinical
Pharmacists with any searches, data collection & medicine safety tasks
- Assist with medication
safety monitoring systems (e.g. high-risk drugs)
- Undertake
patient-supporting roles to ensure effective medicines use and adherence
through shared decision-making.
- Carry out medicines
optimisation tasks including effective medicine administration (e.g.
reviewing inhaler technique), supporting medication reviews and medicines
reconciliation post-discharge and other transfers of care and
synchronising medicines for patient transfers between care settings and
linking with local community pharmacists.
- Assist in the development
of information and communications materials for both patients and staff.
- Support initiatives for
antimicrobial stewardship to reduce inappropriate antibiotic prescribing
working alongside the CCG.
- Provide support in the
clinical aspects of shared care protocols and liaising with specialist
pharmacists for more complex patients.
- Liaise with clinicians and
administrative staff to resolve medicine-related queries.

- Current professional registration GPhC
- Level 3 Diploma in Principles and Practice for Pharmacy Technicians or equivalent
- Minimum 2 years post registration experience
Desirable
- Experience working in primary care
- Enrolled in, undertaking, or qualified from the Primary Care Pharmacy Educational Pathway, Medicines Optimisation in Care Homes, or equivalent
Knowledge and evidence of:
