This is an ideal post for a nurse with a minimum of five years post registration experience.

Main duties of the job

Chronic Disease Management

You will play a crucial role in the proactive management of long-term conditions, including but not limited to diabetes, asthma, COPD, chronic heart failure and hypertension. This involves conducting comprehensive patient assessments, developing individualised care plans, and providing ongoing support and education to empower patients in managing their health. A key aspect of this role is supporting monitoring activities, and patient education while implementing mandatory monitoring schedules as per clinical guidelines, and documenting all monitoring activities in the patient's clinical record.

Treatment Room Duties

Your responsibilities will encompass a wide range of treatment room procedures essential to primary care. This includes cervical cytology, and various injections and immunisations. You will be responsible for vaccine administration, temperature monitoring, and stock rotation, maintaining vaccine storage temperature between +2C and +8C at all times, and monitoring and recording temperatures twice daily. Would care experience is required.

General Practice Support

Beyond these core areas, you will contribute to general practice activities such as health promotion, screening programmes, and providing advice on minor illnesses. Excellent communication skills, a compassionate approach, and a commitment to continuous professional development are essential.

Job description

Job responsibilities

Responsible for meeting the nursing needs and providing nursing care to patients, including the provision of general and specific health assessment and screening to the practice population independently and in consultation with the general practitioners.

To demonstrate the rationale and evidence behind various methods chosen to meet health care needs.

To be involved with practice policy development and to provide nursing input to decisions affecting the care of patients. Keeping up to date clinical management protocols, continuing audit cycles and liaising with lead GP and Practice Manager regarding deadline achievements.

To participate in the administrative and professional responsibilities of the practice team.

To review and implement infection control measures and updating clinicians at clinical meetings.

To participate in education, training, teaching and support of students and staff of all disciplines within the practice.

To maintain professional development by attending courses and study days as agreed with the employer.

To maintain accurate patient records and statistics on the computer system and in the patients notes. Developing on-going IT skills to compliment good standards for dissemination to all clinicians.

Control and maintain medical supplies and equipment used to agreed standards, including the sanitisation of instruments. Keeping inventories for all clinical areas to facilitate stock control methodology. Calibration and care of equipment whilst maintaining records of maintenance, shelf life and replacement need.

To advise patients on general health care and minor ailments, referring to the general practitioners as necessary.

To provide a woman's health service, to include cervical screening and advice and information regarding breast screening; Chlamydia testing, contraceptive pill checks, administration of Depo Provera contraception injections and sign-post for contraception consultation/changes.

To assess appropriately within your capability and nursing grade.

To care and educate patients living with all chronic disease in line with NICE guidelines and the quality outcomes framework. This includes home visits, if needed for housebound patients with chronic disease for their annual reviews and annual immunisations.

Assessment and care of wounds, removals of sutures/clips, administration of injections, venepuncture, receiving and interpreting results where required, following up those who need treatment.

Wound Care plans including the completion of wound assessment documentation/templates and referrals.

Taking of cervical smears, maintaining records, issuing results and following up those patients who need referral. Contributing to the cytology recall system for the health authority and the practice.

To lead on the delivery of the national childhood immunisations programme according to current schedules and any catch up programme The accurate recording of the information in the child health records and to participate in measures (such as education sessions) to improve the uptake of childhood immunisations.

To deal with telephone enquiries/EMIS tasks and provide advice for nursing related queries. This is to include the booking of appointments and ensuring any additional paperwork such as preparation of blood or specimen forms for investigation by the pathology laboratory are completed prior to appointments etc.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Acting on Opportunistic Emis Qof alerts when in consultation with patients.

To chaperone patients where necessary

The undertaking of basic observations, smoking data/smoking cessation advice/signposting, alcohol intake, body mass index, blood glucose monitoring, ECGs, Frailty scoring where required.

Home visits where required and assessments.

Ear assessments, hearing checks and ear irrigation if clinically indicated.
