Job summary
We are looking to recruit a Pharmacist Team Lead to join our established Primary Care pharmacy team working across our Primary Care Network surgeries in the centre of Southampton (St Marys Surgery, Mulberry Surgery and Victor Street Surgery).
Our Pharmacy team consists of 4 pharmacists. It is a well established team that we have supported to grow and develop since the inception of PCNs and the ARRS roles within Primary Care.
The main responsibilities of the role include providing clinical leadership for our pharmacist team, advocating for the role of pharmacists across our surgeries and promoting safe practice (including effective prescribing) and supporting our wider clinical multi-disciplinary team. As well as providing leadership and supervision for the team you will also carry your own clinical caseload and contribute to our senior management team function.
The successful candidate will have completed the CPPE/ Primary Care Pharmacy Education Pathway (PCPEP)for general practice and will be an experienced pharmacist within primary care.
Main duties of the job
The postholder will provide clinical, professional and managerial leadership for the our Primary Care Pharmacist Team. The postholder will work with the Team to develop their skills and competencies and to promote the role of Pharmacists and/or Pharmacy Technicians within our clinical model. The postholder will also undertake clinical duties as a senior Pharmacist with some degree of advances practice and/or specialism within a particular field of primary health care.
The primary care pharmacy team provide direct patient care including supporting patients with long term conditions and undertaking medicines reviews for people with more complex polypharmacy. They provide primary support to general practice staff with regards to prescription and medication queries. They also provide senior support to the prescriptions team, deal with acute prescription queries, and medicines reconciliation on transfer of care. They will develop systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of the patient in our practice.
All staff will have planned specialist CPD with protected time for learning.
