We are looking to recruit a Primary Care Pharmacy Team Lead to join our established Primary Care pharmacy team.
Our current Pharmacy Team Lead is looking to refocus her job plan and as such we are recruiting for the right individual to replace them. Our Pharmacy team is well established and, as a measure of their success, as we move into new PCN arrangements we are excited to be growing the capacity and capabilities of the team.
The main responsibilities of the role include providing clinical leadership for our pharmacy team, advocating for the role of pharmacy within our emerging clinical model, promoting good practice (including effective prescribing) and supporting our wider clinical multi-disciplinary team. As well as providing leadership and supervision for the team you will also carry your own clinical caseload and contribute to our senior management team.
The successful candidate will have completed the CPPE Clinical pharmacists in general practice education pathway and will be an experienced, senior pharmacist with Advanced Practice and/or specialism in a particular area of primary care.
Main duties of the job
The postholder will provide clinical, professional and managerial leadership for the our Primary Care Pharmacy Team. The postholder will work with the Team to develop their skills and competencies and to promote the role of Pharmacists and Pharmacy Technicians within our clinical model. The postholder will also undertake clinical duties as an senior Pharmacist with some degree of advances practice and/or specialism within a particular field of primary health care.
The primary care pharmacy team provide direct patient care including supporting patients with long term conditions and undertaking medicines reviews for people with more complex polypharmacy. They provide primary support to general practice staff with regards to prescription and medication queries. They also provide senior support to the prescriptions team, deal with acute prescription queries, and medicines reconciliation on transfer of care. They will develop systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of the patient in our practice.
All staff will have planned specialist CPD with protected time for learning.
