Job summary

We have an exciting opportunity for a Practice Nurse to join our practice team.

We are two busy high achieving town centre practices looking for a practice nurse due to staff progression and relocation.

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

About us

We currently have a team of 5 practice nurses and 2 health care assistants working together across two practice sites in Cirencester.

The nursing team are also supported by 7 GP partners and 5 salaried GP's, there is also experienced management, reception and administration teams to further support your role.

Cirencester Health Group is a SystmOne practice with over 13,000 patients.

Job description

Job responsibilities

The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  1. To act as an Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager

  2. To produce the IPC annual statement in conjunction with the practice manager

  3. To develop, implement and embed health promotion and well-being programmes

  4. To implement and evaluate individual treatment plans for chronic disease patients

  5. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

  6. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

  7. To provide wound care (ulcer/Doppler etc.) to patients

  8. To provide travel medicine services

  9. To request pathology services as necessary

  10. To process pathology results as required

  11. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

  12. To maintain accurate clinical records in conjunction with extant legislation

  13. To ensure read codes are used effectively

  14. To maintain chronic disease registers

  15. To develop, implement and embed well woman clinics

  16. To develop, implement and embed well man clinics

  17. To chaperone patients where necessary

  18. To assist GPs with minor surgery when required

  19. To prioritise health issues and intervene appropriately

  20. To support the team in dealing with clinical emergencies

  21. To recognise, assess and refer patients presenting with mental health needs

  22. To implement vaccination programmes for adults and children

  23. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

  24. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

  25. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

  26. To support the clinical team with all safeguarding matters in accordance with local and national policies

  27. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

  28. To deliver opportunistic health promotion where appropriate
