Job Purpose

Broomfield Park Medical Centre have an amazing opportunity for a new role to help provide extra support to deliver efficient repeat prescribing processes thus reducing prescribing waste and delivering better quality for patients and the Practice.

In addition to the above, the post holder will also support with practice administration. This will include, registrations, deductions etc.

Main Duties and Responsibilities

Improve the quality of repeat prescribing and medication review systems.

  • Follow and implement practice policies and procedures around prescribing
  • Review and manage an efficient repeat prescribing system using EMIS Web and EPS
  • Provide training and guidance for other practice staff on systems/procedures for repeat prescribing, ensuring adequate cover/expertise is available during holidays/absence
  • Ensure medication review systems are set up and up to date
  • Develop a system for dealing with and reviewing care home medication orders and orders that come direct from a pharmacy on behalf of a patient
  • Assist and encourage patients to order prescriptions through the NHS App.

Improve safety in medicines management by reducing medication errors.

  • Ensure medication reviews are completed when due for patients with a safety net system in place for those that are missed, reviewing as necessary
  • Ensure all hospital discharge letters are dealt with in line with the practice medicines reconciliation policy
  • Ensure clear dosing instructions appear on all repeat prescriptions (with certain exclusions as advised by the medicines management team)

Reduce medicine wastage.

  • Highlight over and under ordering of repeat medicines to the prescriber
  • Ensure prescription medicine quantities are synchronised
  • Remove from a patients repeat list any unnecessary repeat prescriptions that have not been ordered for an agreed length of time

Make savings by reducing/containing costs.

  • Realise simple approved switches where appropriate as advised by the medicines management team
  • Realise dose optimisation opportunities as advised by the medicines management team
  • Promote the use of Scriptswitch in practice and encourage clinicians to act on scriptswitch reports that are generated by the medicines management team

Improve patient and staff satisfaction.

  • Interface with patients and patient groups where appropriate
  • Lead for innovation around practice medicines management
  • Provide training and guidance to other practice staff, and promote communication throughout the practice team

Office administration

  • Supporting with registering new patients.
  • Supporting with removing inactive patients (deductions).
  • Input data correctly onto the clinical system.
  • Scanning of patient related documentation and attaching scanned documents to patients healthcare records.
  • Code and log relevant information in EMIS.
  • General office administration.

Other Duties

  • To support health campaigns being run by NHS Coventry as appropriate
  • To conduct simple audits as directed by the medicines management team that are of interest to the CCG/Area Team
  • To attend mandatory training sessions as directed by the medicines management team basis and to attend adhoc sessions as advised
  • To share best practice with other practices and encourage collaborative working
  • To liaise with MMT leads and act as the link person between the medicines management team and the practice
  • To collate and record data as advised by the medicines management team to measure progress
  • To oversee patient access
  • To assist with prescriptions at the University of Warwick Health Centre (branch site) during the initial few days of registration around early October
  • The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures.
  • The post-holder will support the equality, diversity and rights of patients, carers and colleagues.
  • The post-holder will participate in any training programme implemented by the practice as part of this employment.
  • The post-holder will strive to maintain quality within the practice.

  • Ability to be flexible and adjust to changing service requirements.
  • Confident and pleasant telephone manner.

Desirable:

  • Experience working with GP Clinical Systems for example: EMIS Web, INPS Vision.
  • Knowledge of medicines management relating to primary care.
  • Knowledge of role of other healthcare professionals.
  • Good numerical skills.
  • Willingness to travel to our partner surgeries within Coventry as and when needed.
