Job summary

SAPA 5 Primary Care Network are looking for a Care Coordinator to join our Personalised Care Team.

Care Co-ordinators focus on providing personalised care based on local needs and health priorities.

They work as part of a multidisciplinary team to deliver well-coordinated care, especially to vulnerable patients or those with complex health issues.

This job requires collaboration with GP practices, the Local Authority and other organisations to ensure patients get the best care at that time.

This is a very patient facing role designed to support those in the local community.

Main duties of the job

  • Facilitate and ensure the effective delivery of patient-centred, personalised health and social care plans for patients, monitoring progress and reporting outcomes, contributing to patient reviews and care planning within appropriate time frames and clinical skills
  • Explain the management of patient pathways to clinical staff, liaising between services and service users
  • Work closely with all relevant care agences to ensure a coordinated patient care plan, without requiring a further referral from the GP
  • To provide advice, guidance and practical support to individuals and their families/carers to ensure they are accessing the right care, at the right time, both clinically and socially
  • Ensure that a proper handover of care between different settings has taken place
  • Collect data on patients/carers for recognised outcome measure and document for service interpretation. Ensure all patient notes are updated to reflect any changes
  • Maintain accurate records and statistical returns as required by the ICB, including providing patient-related information for entering into Clinical Reporting Systems
  • To be committed to ongoing professional development, including safeguarding and mental capacity training, and contributing to PCN projects where required

The Care Coordinator will be part of our growing Personalised Care Team, which includes Care Coordinators, Social Prescribing Link Workers, a Mental Health Occupational Therapist, Health and Wellbeing Coaches and a Specialist Diabetes Dietitian. This team works together regularly to support each other to address patient needs.

The post holder will be required to work in different practices within the area so will require a degree of flexibility, access to a car to deliver a range of duties, including home visits, that may vary as the service develops. This is a very patient facing role, to support those in the local community.

The role involves helping to reduce health inequalities, particularly for people with long-term conditions like learning disabilities, dementia, frailty, and cancer.

The care coordinator will support patients to understand and manage their own health needs and to navigate the different health and social services.

This also includes coordinating the work of healthcare professionals and non-clinical staff, including volunteers involved in the care of patients registered at GP practices within the wider PCN population as well as cancer patients.

Desirable

  • Qualification in health or social care allied profession or Health Care Assistant Role
  • NVQ Level 3/Diploma in Health & Social Care

Experience

  • Communication and relationship building
  • Experience in health or social care
  • Experience of developing relationships with a wide variety of people

Desirable

  • Experience of health and social care assessments

Skills and Knowledge

  • Proven ability to recognise and manage risk
  • Knowledge of the needs of vulnerable adults, safeguarding and the associated legislative framework
  • Understanding of basic health and social care terminology
