Job summary

Are you an experienced nurse prescriber that is looking for a new challenge.We are a training practice with a list size of 17000 patients across West Lancs. 2 branches in Skelmersdale, 1 branch in Southport and 1 branch in Ormskirk. We have a huge family of clinical staff. We are an innovative practice that delivers all national and local enhanced services and offer an abundance of training opportunities. Part time, full time /flexible hours to suit will be considered. Our approach is innovative and forward-looking, and dedicated to excellent clinical outcomes.We are purposeful in our efforts to manage our clinicians workload and ensure sustainability, having developed a diverse and large team of support staff over recent years dealing with many admin tasks including referral, prescriptions and path links.We embrace technology to enable more effective and efficient ways of working. We have introduced an online triage system for our patients allowing our patient to complete an on-line form detailing the problem. It has significantly improved access for our patients and reduced our workload as more conditions can be safely signposted to the appropriate clinician or service.

Main duties of the job

To provide and maintain high quality nursing care to the practice population.

Delivering sensitive, and responsive care, ensuring the ethnic, linguistic and cultural needs are taken into account.

To promote the practice as a learning organisation including staying abreast of policy, guidelines and evidence and communicating and integrating this into practice

About us

Our Practice family consists of 7 GP's, GP Registrars, 2 ANP's, Nurse Prescribers, Practice Nurses, Pharmacists, Community Paramedic, and HCA's. Our administrative team - 17 receptionists which includes a head receptionist at each location, 3 clinical coders, one scanner, data quality lead, 2 deputy Practice Managers, Office Manager and Practice Manager. We are an innovative practice that delivers all national and local enhanced services and offer an abundance or training opportunities. We have a whole practice study day every month where the whole of our team gets involved.

In addition to our core general practice work we are a committed member of our local PCN, working Collaboratory to improve patient access within our local area.

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the practice population across all Beacon Primary Care sites. The post holder is accountable for contributing to the nursing service delivery. Clinically, the focus of the role is the delivery of evidence-based practice for all patients whilst focusing on individuals with long-term conditions offering management and preventative interventions. As an autonomous practitioner the candidate will be responsible for the care delivered, whilst also demonstrating critical thinking and skills in decision-making. As a V300 Independent Nurse Prescriber you will be responsible for prescribing effectively and appropriately within your scope of practice. The candidate will work collaboratively with the whole general practice team to meet the needs of patients, providing leadership and ensuring care within practice policies and procedures.

Key responsibilities Clinical practice

  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being,
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
  • Assess, diagnose, plan and evaluate treatment/interventions for patients presenting with an undifferentiated diagnosis, referring to secondary/community care as appropriate.
  • Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
  • Prescribe and review medication for therapeutic effectiveness appropriate to patients needs and in accordance with national and local guidelines and within own scope of competence.
  • Work with patients in order to support adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
  • Support and manage health needs of women presenting for family planning and cervical cytology consultations (if appropriately trained)
  • Implement and participate in vaccination and immunisation programmes for both adults and children

  • Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
