Job summary

Are you an Advanced Nurse Practitioner that is looking for a new challenge. We are a training practice that has a list size of 17000 patients across 4 locations in West Lancashire. Two Practices in Skelmersdale, one Practice in Banks Southport and one in the old historic town or Ormskirk. We have a huge family of clinical staff. We are an innovative practice that delivers all national and local enhanced services and offer an abundance of training opportunities. Part time, full time /flexible hours to suit will be considered.

Main duties of the job

The Advanced Nurse Practitioner is responsible for managing and providing care for the presenting patient with undifferentiated undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long-term conditions in line with existing practice and national policies and guidelines.

You will work within your own professional competence in accordance with the NMC Code of Conduct. The team member will provide a service which is flexible, sensitive, and responsive to the health needs of those living and working in the area, ensuring the ethnic, linguistic and cultural needs are taken into account.

About us

Our Practice family consists of 7 Gp's, GP Registrars, 3 ANP's, Nurse Prescribers, Practice Nurses, Pharmacists, Community Paramedic, and HCA's. Our administrative team - 17 receptionists which includes a head receptionist at each location, 3 clinical coders, one scanner, data quality lead, 2 deputy Practice Managers, Office Manager and Practice Manager. The Health and Well Being of our staff matters immensely to us, so have recently appointed a Health and Well-being lead and Champion. We are an innovative practice that delivers all national and local enhanced services and offer an abundance or training opportunities. We have a whole practice study day every month where the whole of our team gets involved.

Job purpose

The Advanced Nurse Practitioner is responsible for managing and providing care for the presenting patient with undifferentiated undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long-term conditions in line with existing practice and national policies and guidelines.

You will work within your own professional competence in accordance with the NMC Code of Conduct. The team member will provide a service which is flexible, sensitive, and responsive to the health needs of those living and working in the area, ensuring the ethnic, linguistic and cultural needs are taken into account.

Key responsibilities

Clinical

1. To provide direct access to patients seeking primary health care. Both face to face and on the telephone. To be involved with On Call sessions based on rotation.

2. The post holder is required to work within the policies and procedures of the practice and with due regard to the NMC Code

3. Assess diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, referring to secondary or community care as appropriate.

4. Assess diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs, to maintain extensive knowledge of chronic disease management and their processes and management

5. Proactively identify, diagnose, and manage treatment for patients at risk of developing long term conditions

6. Prescribe and review medication for therapeutic effectiveness appropriate to the patients needs and in accordance with national and practice protocols and within scope of practice

7. Work with patients and pharmacists to support concordance to treatment and provide information and advice on prescribed or OTC medication- side effects and interactions

8. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations

9. Support patients to adopt health promotion strategies and self-care, referring as appropriate

10. To prioritise and manage our care homes, conducting ward rounds, managing complex needs, liaising with GPs, voluntary sector, district nurses or community matron as appropriate

11. To prioritise and develop the management of housebound patients Work closely with our GPs e.g. in audit and analysis of audit in relation to CCG.

Developing models of good practice from internal and external sources to improve clinical care within the practice team and hence help in reducing admission, prescribing and duplication of work.

Communication

1. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

2. To ensure that patients and carers who are at risk because of cultural and or language barriers are helped to understand and make use of the various services available

3. Maintain effective communication within the practice environment and with external stakeholders

4. Endorse practice policy on confidentiality

Delivering a quality service

1. Recognise and work within own competence and professional code of conduct as regulated by the NMC

2. To use the practice clinical computer to ensure all records of patient episodes are accurate, complete, and become permanent

3. Deliver care according to NSF NICE and local guidelines and evidence-based care responding to initiatives as appropriate

4. Assess effectiveness of care delivery through self and peer review

5. To initiate or contribute to the development and review of practice protocols

6. Support and participate in shared learning across the practice and wider organisation

7. Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory health procedures and local guidance
