Job summary

We are looking for an enthusiastic, self-motivated, dynamic Practice Nurse to join our friendly multidisciplinary team . The candidate will require recent experience within a Primary Care Setting, be adaptable, highly motivated, relish a challenge and want to develop an ongoing career in Primary Care. Accredited training and Knowledge, skills and experience of working with chronic disease management is highly desirable.

Main duties of the job

The purpose of the post is the delivery of safe, high quality primary care nursing services to the practice population. Working within the NMC Code of Conduct, the post holder will within the nurse team, ideally using previous primary care experience provide advanced nurse interventions and influence development of nursing service.

Mid Sussex Health Care is a high performing, innovative, well established training practice. We have achieved a consistent CQC rating of Good in all areas, and rate highly in patient satisfaction surveys. The Practice is situated in close proximity to the South Downs National Park, Brighton and the coast and there are excellent road and rail connections to Brighton, Crawley, Gatwick and London.

We currently have 11 GP partners, 1 Business partner, 1 salaried GP and 10 members in our nursing team. The practice also benefits from pharmacist time through our PCN and Social Prescribers and Care co-ordinators, Health and Wellbeing coaches and child and Young Adults Social prescribers and the mental Health support team. All of our clinicians are supported by a well organised and efficient administrative team.

Our GP partners are engaged in roles including the PCN (Clinical Directorship), PCN Cancer Leadership, GP Appraisers, and Education with a GP Training Program Director, 4 GP Trainers and 1 FY2 Clinical Supervisors.

Job responsibilities

Clinical practice

  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
  • Proactively identify and manage treatment plans for patients at risk of developing a long-term condition and communicate with GP
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
  • Work with patients in order to support adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Advise, support and administer vaccinations where appropriate for patients travelling abroad
  • Promote and deliver evidence-based care for patients presenting with aural conditions
  • Undertake cervical cytology in line with local/national screening policy

  • Anticipate barriers to communication and take action to improve communication
  • Estimate and maintain effective communication with individuals and groups within the practice environment and with external stakeholders
  • Act as an advocate when representing patients and colleagues
Delivering a quality service
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality
  • Deliver care according to NSF, NICE guidelines and evidence-based care
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation

  • Registered first level nurse Minimum 2-3 years post registration experience.
  • IT Literate.
  • Excellent communication skills (verbal and written).
  • GCSE grade A to C in English and Maths
  • Qualified smear taker (current)
  • Competent in child immunisation

Desirable

  • Diploma level qualification in the care of Asthma and COPD or/and Diabetes
  • Experience of SYSTM1 clinical system.
  • Experience and evidenced training in travel health and immunisations.
