Job summary
Towcester Medical Centre are looking for an Advanced Clinical Practitioner to join our friendly team.
We are looking for an experienced Independent Prescriber from a Nursing or Paramedic background who can provide care in GP Practice, including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
The successful candidate will have the opportunity to work autonomously whilst enjoying the support of our GPs when needed.
Main duties of the job
Responsible for attending to patients presenting with a range of acute and chronic medical conditions, providing expert advice.
History taking, diagnosing, treating and monitoring patients within the Primary Care setting.
Deliver patient care for chronic disease management.
Maintain accurate clinical records using SNOMED codes.
Support the team with clinical emergencies.
Practice in compliance with code of professional conduct.
Independent Prescribing within areas of competence.
Initiate evaluate and modify a range of interventions including prescribing medications, therapies, lifestyle advice and care

Take an active role in clinical supervision and reflective practice

Referring patients to secondary care as required.
Maintain accurate clinical records including SNOMED coding
