Arch Health CIC is a values driven, non-profit health care
provider which delivers primary care to homeless and vulnerably housed people
in Brighton & Hove. The service is staffed and led by a dedicated local
team and was rated outstanding in all 5 areas of a CQC inspection in January
2019.
In addition to a specialist GP surgery, we also provide outreach
services into the Royal Sussex County Hospital and a variety of community
contexts which serve people who are without secure accommodation.
We are looking to appoint a prescribing ANP or Paramedic to lead the
set-up and delivery of an enhanced outreach offer which will deliver healthcare
on the streets, in day centres, hostels and temporary accommodation. We also
hope that a mobile treatment centre will form part of this project. The project will be supported by a specialist GP with regular clinical supervision and will have the support of the wider multidisciplinary network and the Arch team.
Main duties of the job
This is a new project and leadership of it will require a high level of
flexibility, drive and ambition in order to achieve the benefits which we
anticipate for our patients. You’ll work closely with and manage a GP, Health
Engagement Worker and other members of the Arch Team to ensure the success of
the project.
Our ideal candidate will be a highly self-motivated leader who is
organised, driven, open and caring. They will be able to think on their feet,
have great problem-solving abilities and be comfortable with the uncertainty
which inevitably accompanies a new project in its infancy. Most importantly
they will be dedicated to the delivery of the highest possible standards of
care for people who are living without homes in Brighton & Hove.
Job Purpose:
1. PATIENT FOCUS: delivering patient care on the streets and in
community contexts.
2. PROJECT FOCUS: providing leadership to the
set-up and delivery of the project including all practical and administrative
aspects.
3. PARTNER FOCUS: managing the set-up and
delivery of a new clinical outreach project in Brighton & Hove ensuring strong integration into the wider homeless health network.

Clinical Leadership:
- To independently manage patient care across various contexts, prioritising work effectively
- To effectively engage patients from a variety of backgrounds
- To independently undertake effective comprehensive holistic health assessments for patients including independent assessments of risk
- To take time to listen to patients who may have highly distressing personal circumstances and stories to understand their needs
- To utilise strategies to improve communication with patients e.g. accessing Language Line, taking time to explain leaflets and letters etc
- To utilise strategies to work to minimise the impact of disability on patients e.g. identifying the need for a walking aid, or new wheelchair etc
- To provide subsistence support to patients as required in line with service policy
- To make appropriate referrals for patients as necessary
- To proactively identify the need for e.g. mental capacity assessments, safeguarding referrals, challenges to NHS charging decisions within the caseload and take action as required
- To record all work undertaken with patients in an accurate and timely manner in all relevant clinical databases, enabling effective integrated care, and the collection of monitoring and evaluation data
- To clinically advocate and negotiate on behalf of patients with partners, health colleagues and hospital staff without generating conflict

Administrative: - To maintain accurate patient records, entering onto the practice system the appropriate agreed codes, using agreed standards of data entry, templates and protocols, completing all documentation, computerised and paper-based, as is required for good patient care.
- To maintain robust systems of data collection, audit and review of all clinical activities, protocols, care pathways and clinical systems, liaising with fellow colleagues in the practice and in the local and national health system as needed to insure good patient care and service is maintained and to satisfy the standards of external regulatory bodies such as the CQC etc.
- To participate, in partnership with other clinical and managerial staff, the necessary data collection for QOF and other incentive schemes as needed by the practice, prompting other members of the team as needed for assistance in gathering the required data.
- To create and maintain in cooperation with other nursing and admin colleagues a system of clinical stock and equipment maintenance, calibration, and supply; working closely with the practice manager to insure cost efficient purchasing of perishable and expensive items.
- To maintain all documentation needed to carry out your clinical duties.
- To participate Infection Control procedure and protocols and their implementation within the practice.

- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
