Job summary

The PCN health engagement project takes a grassroots and community approach to promoting health improvements amongst the East and Central Brighton population.

Our ideal candidates will be organised and able to work with diverse involved groups clinical staff, patients, community groups in order to deliver the health improvement outcomes set out by the PCN directors. They will also have excellent written and verbal communication skills with the ability to analyse and convey complex information with clarity, as appropriate for the audience. They will be comfortable working with people from different backgrounds and in varied situations and contexts.

Main duties of the job

Patient Engagement Workers will be responsible for the project management of a distinct set of initiatives designed to early identify possible clinical conditions and risks, to improve the health and wellbeing, and hence the life chances, of East and Central Brighton residents. This incorporates a number of different public health priorities: vaccination; early diagnosis of bowel, breast and cervical cancer; chronic obstructive pulmonary disease; diabetes; health checks.

About us

Arch Health CIC is a social enterprise delivering primary care to homeless and vulnerably housed patients. We are looking for two workers to support the Primary Care Network in East and Central Brighton.

East and Central Brighton PCN consists of 9 GP surgeries generally covering the Whitehawk, Queens Park, Kemptown, Hanover, Lewes and London Road areas of Brighton.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

  1. To work as part of the GP surgery teams to:

  • Undertake clinical system searches to identify patients
  • Ensuring proactive contact of non-responders for screening and encouragement to attend
  • Ensure a range of appropriate cancer information/signposting is available to all patients
  • Ensure an integrated and personalised approach to patients identified as requiring screening.
2. Undertake outreach and community development activities promoting health opportunities with community groups and individuals, especially within hard-to-reach groups, facilitating engagement and increasing take-up.

3. Responsible for utilising persuasive, motivational, negotiating and empathetic skills when communicating with patients.

4. Involvement in patient and participation groups to get feedback from patients

5. Ensuring practice participation in local and national awareness health promotion campaigns, in liaison with Public Health England, to ensure equal access to health promotion activities and information for all groups.

6. Attendance at educational training relevant to role including behaviour change.

7. Collect, collate, evaluate and report information, maintaining accurate patient records as specified by the practices procedures and data quality standards.

8. Work collaboratively with other professionals and organisations to ensure patient needs around health screening or support for health screening are met.

9. To ensure that all complaints and compliments received are recorded in line with organisational policy.

10. To recognise the importance of safeguarding procedures within the project, and to ensure that any concerns, disclosures or allegations of abuse are immediately and correctly reported.

11. To embed equality, diversity and inclusion best practice into all aspects of work.

12. To work to the requirements of the organisation's quality standards, and other service-specific quality accreditations as required, abiding by organisational ethos and principles

13. To attend and participate in regular supervision, appraisals, training and other internal meetings.

14. At all times provide a caring service and to treat everyone in a courteous and respectful manner.

15. Carry out duties and responsibilities in compliance with the Health and Safety Policies and Statutory Regulations.
