The purpose of this exciting new role is too support our Pharmacy technicians and prescriptions team, on a temporary 12 month basis with the view to continue if it is compatible with the needs of the business. Main duties of the job
Have telephone or face to face conversations with patients
who have medication queries and resolve the issue with the support from our
multidisciplinary team
Processing repeat and acute prescription requests
Regular liaison with community and hospital pharmacists as
well as consultants
Support the achievement of the practices prescribing quality
and outcomes framework targets
Maintain a broad understanding of the work of the pharmacy
team and the practice as a whole
Work to agreed protocols set out by the GP Partners while
following Cheshire and Merseyside ICB policies

Penketh Health Centre is an innovative practice focused on providing a high level of care to our population of 12500 patients. Our large friendly team consists of 6 GP partners, 3 salaried GP's, 3 advanced nurse practitioners, 4 practice nurses, 2 pharmacy technicians, 2 healthcare assistants, 3 Managers, 11 Patient Advisors. As well as a team of different professions employed through the ARRS scheme via Central and West Warrington PCN who we are proud to be a part of.

Job responsibilities
- To work as part of the practice team providing support both clinical and administrative teams.
- Work with the Prescriptions team, Pharmacy Technicians and PCN Clinical Pharmacist where required.
- Medicines reconciliation post discharge and on other transfers of care, actioning changes where appropriate within competencies.
- Provide support to GPs and other members of the primary healthcare team.
- Undertake reviews to ensure adherence to monitoring and shared care protocols.
- Telephone or face to face conversations with patients who have medication queries and resolve the issue with the support from our multidisciplinary team
- Day to day handling of prescription queries within area of own competence.
- Review nursing home prescription requests.
- Carry out housekeeping whilst in patient records
- Promotion of electronic repeat dispensing and online ordering.
- Assisting in advising clinicians of alternative meds when notified item(s) are unavailable.
- Assist in practice response to complaints or significant events.
- Liaise with other pharmacy stakeholders e.g. Medicines Management Team, community pharmacy and hospital pharmacy teams.
- Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldecott Guidelines; and the Confidentiality Code of Conduct.
- To assist with the development and review of medicines audits utilising the TPP GP practice clinical systems to conduct the audits.
- Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within the practice over an appropriate time frame and to advise on any medication changes that occur as a result.
- Action clinical letters liaising and seeking support and guidance from the GPs when required.
- Query any unlicensed medication requests with the GP Partners
- Promote cost-effective, safe, evidence based prescribing in accordance with national and local guidance.
- To undertake training and development, to ensure the required knowledge and skills for practice based work.
