Job summary
We have an exciting
opportunity for an individual with experience of working in substance use
and mental health to join our team. The Foundations HILT service is based in
the Urgent and Emergency Care Department within James Cook Hospital. The role
will be to see and assess any patient accessing the hospital with drug and / or
alcohol issues for engagement and support, referring into community services
for support where needed. The ideal candidate will be confident working within
a hospital setting and have experience with mental health and substance use.
The service is currently a 7 day service which covers weekends and Bank holidays. Full time employees are expected to work their contracted hours on a rota basis, covering the service opening hours of 8am and 8pm, working a minimum of 4 days a week.
Main duties of the job
JOB PURPOSE
To ensure patients are
aware of the risks associated with their substance use and support
them do develop strategies to reduce the associated harm of their substance
use.
To support patients to
access appropriate health care supporting them to address often complex health
needs and reduce incidents of drug related death.
Liaise and
communicate with multi-agency staff.
To identify, assess
and refer individuals who need substance use support (including alcohol).
To support the
facilitation of hospital to home detox processes.
To deliver prevention
focused interventions.
To build and maintain
links with MACE (Middlesbrough Alcohol Centre of Excellence).
To liaise with
community substance use services.
