Job summary

We are a rural two site dispensing Practice in North Norfolk with a patient list size of approximately 9100 patients with surgeries in the towns of Reepham and Aylsham.

We are pleased to offer this exciting new opportunity for an enthusiastic and experienced individual to join us. You will join an established, multidisciplinary team responsible for delivering high quality medicines management and clinical pharmacy input to our patients.

Main duties of the job

Provide medication review services to patients via clinics in the practice, and residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.

Assist partners with the appropriate monitoring and management of their prescribing budgets.

Prepare evidence-based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.

Help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.

To update and maintain accurate patient medication records on the practices SystmOne clinical computer systems

Advise the primary health care team on the safe and secure handling of controlled drugs and other medicines

To be responsible for the updating of all PGDs

To action all safety alerts

To be responsible for all stock ordering

To be the lead in the practice for PQS

Authorise daily EPS Requests

Respond to daily medication queries from patients

About us

We are a friendly, established Practice with four GP partners and a wide supporting clinical and administrative team. The practice aims to provide great clinical care to their patients within a supportive, sustainable environment for the team. We are a dispensing practice, and part of the NN3 North Norfolk Primary Care Network, and has a close working relationship with the other practices in North Norfolk. The practice is part of the larger Norfolk and Waveney ICB.

Job description

Job responsibilities

  • Help the practice deliver QOF and LCS
  • Work with the clinician and administrative team to support patients to address medicine adherence, reducing the wastage and overuse of medicines
  • Monitor patients’ blood results, focusing on potential harm that can come from long-term use of medicines or poor compliance
  • Review patients on complex medicine regimens/ with ongoing health problems
  • Support doctors by highlighting changes in NICE guidelines
  • Triage and manage common ailments
  • Take part in multi-disciplinary case reviews
  • Deal with medication for patients recently discharged from hospital, including liaison with hospital, community and primary care colleagues
  • Work with the practice team to deliver repeat prescription reviews, especially for care home residents the frail and vulnerable patients

Person Specification

Experience

Essential

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council. Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio.
  • Member of the Royal Pharmaceutical Society (RPS)
  • Must hold an independent prescribing qualification
  • May hold or be working towards a postgraduate pharmacy qualification

  • Experience of working in a GP Practice
