Job summary
Due to recent retirement, Swallownest Health Centre are looking to appoint a highly motivated, enthusiastic, dedicated, passionate and caring Lead Practice Nurse to provide cover 30 to 37.5 hours per week. The successful candidate will have experience in chronic disease management and ideally Line Management. Experience of Primary Care is essential.
Practice Nurses within our practice are an important part of the clinical team. The successful candidate will lead our dedicated Nursing team and support GPs and the Nursing team in the care of practice patients to include treatment, preventative care, screening and patient education.
It is essential that you have a flexible approach to your working hours in order to fit with the demand/space available within the practice, which is limited.

Main duties of the job
To be responsible for the line management of the practice nursing team. Working as part of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively and efficiently in line with NHS contract requirements. The lead practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, as well as supporting the practice management team in the reviewing and delivery of Health & Safety requirements along with clinical policy and procedure.
Provide nursing treatments along with general and specific health screenings, minor ailments and chronic disease management to patients in participation with general practitioners or independently to agreed protocols.
Undertake the collection, storage and dispatch of pathological specimens including intravenous blood samples, swabs, smears etc.
Use of a computer to manage daily workload; e.g. email, messaging, basic word and excel however clinical system training will be provided.
