Job summary

An exciting opportunity to be part of an innovative practice, which is an active participant in the primary care network and works very closely with the local GP Federation One Health Lewisham. We are looking for a clinician ideally with primary care experience. They should be enthusiastic and motivated to develop their knowledge and skills to improve patient care in general practice.

Main duties of the job

The post holder will work as part of a multi-disciplinary team and provide clinical leadership for the Practice, taking responsibility for Practice priorities including; Structured Medication Reviews, areas of chronic disease management, medicines optimisation, quality improvement, the quality and outcomes framework and other enhanced services within the practice and PCN.

The clinician will work within their skillset and experience to provide support to practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage GP workload. The clinician will work very closely with current clinical pharmacist and GPs to improve patient care.

About us

We are a large practice with a patient population of 12000. We have two GP Partners, one salaried GP and four regular Locum GPs, a Practice Nurse, Health Care Assistant and a team of Receptionists, Administrators and Secretaries.

The Practice relies on a team of hardworking, efficient and friendly Staff with a great sense of humour, in this fast paced environment.
