Job summary

Laurel House Surgery understands the significant role an ANP plays within general practice. As such, we are looking for an enthusiastic and motivated individual to join our existing team (8 GPs, 2 ANPs, 2 PAs, 4 RGNs, 2 HCAs, etc.).

Hours: The role can be up to full-time so applicants looking for either full or part-time should apply.

Salary Range: £50-55,000 WTE depending on experience.

The practice is a busy, well established practice based in Tamworth, Staffordshire. The practice has good transport links (i.e. M1/6/42, near train station) so easily accessible to the surrounding areas.

The practice comprises of 2 separate buildings, one a purpose-built Health Centre and the second a building which has been used as a GP practice for a number of decades. The practice has a patient list size of just under 13,000 patients and uses Emis as its clinical system.

Main duties of the job

To work as an autonomous practitioner

Responsible for:

o Attending to patients who wish to be seen by a health care professional

o Diagnosing, treating and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury

o Making an assessment of their needs, based on highly developed nursing skills including physical assessment.

Seeing and advising people in respect of their continuing medical and nursing needs

Patients are to be seen in a timely manner enabling contractual requirements to be met

The ANP will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. There will be a requirement to see extra or emergency patients

Responsible for:

The role is flexible and demanding and may be expanded in accordance with practice requirements and the nurse practitioners own scope of practice

The role will involve managing the care of patients who have acute and/or chronic health needs

Participate in the delivery of care to patients through Home Visits

SPECIFIC DUTIES/RESPONSIBILITIES

Clinical Responsibilities: -

Identify, assess and diagnose patients with complex health needs across the practice population

Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs

Prescribe products to meet the needs of patients within the practice population

Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups and individuals

Work with others to change policies to improve health and well-being and reduce inequalities

The implementation and evaluation of evidence-based practice to improve the services offered by the practice Allied Healthcare Team.

Participate in quality assurance, development and clinical governance

Work with other members of the practice team in the implementation of specific aspects of national guidelines

To adopt a holistic approach working with individuals and wider populations to promote health through preventative measures.

Diagnosing and treating patients: -

Triage and treat patients wishing to see a health care professional making any referrals to other members of the primary health care team

Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations

Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary

Manage chronic disease clinics as required keeping the practice up to date with QOF requirements and ensuring high standard of care to this group of patients

Pathological specimens and investigatory procedures: -

Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.

Perform investigatory procedures

Review and report upon clinical results formulating appropriate action plans within the practitioners competencies.

Administration and professional responsibilities: -

Patriciate in and undertake relevant audit to ensure the practice is up to date and patients needs are being met

Participate in the administrative and professional responsibilities of the practice team

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes

Ensure relevant clinical coding is used when and where appropriate

Ensure the clinical computer system is kept up to date with accurate details recorded and amended

Ensure appropriate items of service claims are made accurately, reporting any problems to the Practice Administrator

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice

Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit

Attend and participate in practice meetings as required

Maintain a professional working environment.

Training and personal development: -

Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements

Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments

Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring mandatory requirements are met

If it is necessary to expand the role to include additional responsibilities, training will be given

Develop and maintain a Personal Development Plan

Liaison: -

There is a need to work closely with all colleagues to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the Allied Healthcare Team.

There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care.

Meetings: -

It will be necessary to attend and contribute to various Practice meetings as requested.

Using personal security systems within the workplace according to Practice guidelines
