Job summary
We are seeking an enthusiastic
and experienced Practice Nurse to join our friendly and supportive team on a
part time basis at Stapenhill Medical Centre. This is a well-established GP
surgery serving a diverse population in East Staffordshire.
Main duties of the job
The role would have a particular
focus on Diabetes and Chronic disease management, alongside Travel Vaccinations
and the Childhood Vaccination schedule. The GPN is also responsible for the
delivery of practice nursing services, working as part of the practice
multidisciplinary team, delivering care within their scope of practice to the
patient population, and supporting the management team in the reviewing and
implementation of clinical policy and procedure.

We currently have three full-time Partners, assisted by
three salaried GP's, a Clinical Pharmacist, an Advanced Nurse Practitioner, two
Practice Nurses, one Healthcare Assistant, two General Practice Assistants and
one Care-Coordinator.
With a patient list currently in excess of 9,500 and
growing, and with the development of new services, we are seeking to recruit to
address our future needs.
We are a friendly well-established Practice with high QOF
scores and a Good CQC rating.

Job responsibilities
Primary responsibilities:
- To implement and evaluate individual treatment
plans for patients with diabetes.
- To support clinical staff, including nurses,
GPs and GP trainees with diabetes education.
- Identify, manage, and support patients at risk
of developing long-term conditions, preventing adverse effects on the patients
health.
- Provide routine nursing care to patients as
required in accordance with up-to-date evidence-based practice.
- Provide travel vaccination services.
- Provide chronic disease clinics, delivering
patient care as necessary, escalating to the Duty Doctor as needed.
- Undertake cervical smears and swabs and maintain
smears results log as per guidelines.
- Maintain accurate clinical records in
conjunction with present legislation.
- Maintain chronic disease registers with the
Nursing team.
- Implement vaccination programmes for adults and
children.
- Recognise, assess, and refer patients presenting
with mental health needs.
- Support the clinical team with all safeguarding
matters, in accordance with local and national policies.
- Deliver opportunistic health promotion where
appropriate.
- Within the Nursing team, assisting with waste
management and Infection Control procedures.
Secondary responsibilities:
- Assist GPs with minor surgery when required.
- Chaperone patients where necessary.
- Support the team in dealing with clinical
emergencies.
- Liaise with external services / agencies to
ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately
(ensuring safe practice and the task is within the scope of practice of the
individual.)
- Understand practice and local policies for
substance abuse and addictive behaviour, referring patients appropriately.
- Monitor and ensure the safe storage, rotation,
and disposal of medicines.
- Support junior members of the nursing team,
providing guidance and education.
- Participate in local initiatives to enhance
service delivery and patient care.
- Support and participate in shared learning
within the practice.
