We have an
amazing job opportunity for a Lead Practice Nurse to join our highly effective,
friendly and supportive team on a full or part time basis. Salary is negotiable
and dependent on qualification and experience.
As Lead Nurse you will be supported by the partners, Clinical Care Management Team and a dedicated Nursing team co-ordinator. You will work collaboratively with the whole Primary Care Network (PCN) to meet the needs of patients and provide leadership and direction for the nursing team. Our ideal candidate will have drive, dedication, enthusiasm, and a positive attitude. They will have the ability to work well with all teams across all sites and be open to innovative ways of working.
Main duties of the job
Role Responsibilities:
The key focuses of the role will be training, mentoring and supporting the nursing team to help further develop their skills & to support patients to be healthy, monitoring and treating long-term conditions and proactively supporting health promotion and screening activities.
Skills in chronic disease management (particularly respiratory illnesses) are an essential requirement.
The Lead Nurse will be accountable for nursing service delivery and ensuring the delivery of safe and effective nursing care to the whole practice population. They will lead and manage all the nursing clinical resources, working closely with the Clinical Care Management Team to deliver the PCN priorities. The Lead Nurse is not responsible for HR but will link in with matters such as recruitment and appraisals of the team.
The successful candidate will be expected to:
- Deliver a high standard, safe patient care using specialist practice nurse skills.
- Mentor new nursing staff
- Provide clinical leadership for the nursing team
- Liaise with the Clinical Management Team and Education Leads to make sure the nursing team are fully trained and qualified
- Maintain and review nursing protocols
- Ensure self-development and registration requirements are kept in date and regularly reviewed
- Work with the Management team on matters such as recruitment and appraisals
- Meet CQC standards

KEY RESPONSIBILITIES
- To develop and supervise the nursing team in training, ensuring compliance and development needs are met (including staff reviews and appraisals)
- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and wellbeing programmes
- To work in conjunction with the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance where necessary.To assist the IPC lead in producing the IPC annual statement
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
- To provide routine nursing care (including cervical screening) to patients in accordance with practice policies and protocols
- As required, to diagnose and manage acute and chronic conditions
- To prescribe medication for patients in accordance with PGDs and own scope of practice
- To review patient medications to enhance compliance
- To support the provision of travel vaccination services
- To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
- To ensure accurate, clinical details are recorded at all times
- To ensure read codes are used effectively by all members of the nursing team
- To ensure the appropriate maintenance and use of chronic disease registers
- To ensure compliance with the practice chaperone policy
- To prioritise health issues and intervene appropriately
- To ensure CQC Compliance is met at all times
