Nurse Associate - Winchester East & North Rural PCT Hub

This job is based from Alresford Surgery, Winchester as the Hub location for Winchester East & North Rural PCT Hub

You will be working within our PCT hub covering various GP surgeries that fall within that Hub area. If you are a passionate, motivated team player and want to excel in delivering exceptional holistic healthcare to a diverse patient group alongside of a range of community experts, we want to hear from you.

Previous experience of community nursing is a benefit but not essential as we offer a structured training development programme with formal support, provided.

As a nurse associate, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

Main duties of the job

The post holder will have completed their Nursing Associate training and be registered with the NMC. Be able to work independently under the leadership of registered nurses, working within the sphere of community nursing.

The Nursing Associate will work as part of the Proactive Multidisciplinary Care Hub, delivering high quality, compassionate care whilst undertaking specific clinical tasks under direction of a registered nurse (or other registered health professional), with a focus on promoting good health and independence.
