This role is located in Winchester, Hampshire. Not Andover.

An exciting opportunity has arisen for a Patient Care Co-ordinator to join the Winchester City Primary Care Network (PCN) Wellbeing Service.

This role will involve working across the 3 practices that form Winchester City PCN; the Friarsgate Practice, St Pauls Surgery and St Clements Surgery. The Wellbeing Service is a relatively new service for patients in primary care and gives the opportunity for you to take part in the development and growth of this new and exciting venture.

The Wellbeing Service is comprised of a multidisciplinary team, currently consisting of Social Prescribers and the Primary Care Mental Health Team (counsellor, wellbeing practitioners, peer support worker, occupational therapists, physical health & wellbeing coach and CBT practitioner). The post holder will have excellent communication skills and will be an integral member of the Wellbeing Service, supporting not only PCN colleagues but also the wider practice teams, community and secondary care services.

Main duties of the job

The post holder will work autonomously to manage their caseload and will be able to assist patients in accessing appropriate health and social care provision. The Patient Care Co-ordinator will encourage patients to achieve better mental and physical health through encouragement and support with accessing services and benefits in order to be able to achieve their goals.

Cover for colleagues who administrate and oversee the weekly mental health and frailty multidisciplinary team meetings (MDTs) will be required on occasion. The post holder will be able to recognise limits to own capabilities and will be confident in referring to other health professionals or regular MDT meetings when this is in the best interests of the patient.

The successful candidate will have experience working in a similar role within a health care environment and will be able to evidence that they can manage a caseload of patients.

PCN Winchester works collaboratively with the three Winchester City GP practices to provide additional healthcare services that meet the needs of the patients.

Working closely with the patient and their clinician or other healthcare professional, the PCC co-ordinates patients healthcare and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The PCCs role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and pharmacy technicians.
