Proactive Care Team Nurse - Winchester Rural East & North

Job summary

We are looking for an exceptional nurse to join our team at Mid Hampshire Healthcare which covers the Winchester City North and Rural East area.

Previous experience of community nursing is a benefit but not essential as we offer a structured training development programme with formal support, provided. If you choose to join us here, many of your new colleagues will have joined us from acute services and found community nursing to be the exciting and challenging next step in their nursing career.

As a community nurse, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

Main duties of the job

The main purpose of this role is to provide proactive clinical support to vulnerable patients and those with complex physical or mental health needs who are considered to be at high risk of future hospital admission within the Winchester and Andover Primary Care areas. The aim is to improve their quality of life and healthcare experience and therefore reduce the risk of emergency hospital admission.

MHH is GP federation supporting 18 GP practices in the Mid Hampshire area. As listed, this role covers Winchester East and North Rural as part of the PCT team.
