This is an exciting opportunity to join our growing team in Andover PCN.
The role of Primary Care Network Occupational Therapist has an essential role
in improving the healthcare we provide to our patients.
We are recruiting an OT to manage a varied and complex caseload
providing assessment and intervention to patients aged 16 and over who are
identified as needing occupational therapy.

Main duties of the job
Primary Responsibilities
assess, plan, implement, and evaluate treatment
plans, with an aim to increase patients productivity and self-care;
work with patients through a shared-decision making
approach to plan realistic, outcomes-focused goals;
undertake both verbal and non-verbal
communication methods to address the needs of patients that have communication
difficulties;
work in partnership with multi-disciplinary team
colleagues, physiotherapists and social workers, alongside the patients'
families, teachers, carers, and employers in treatment planning to aid
rehabilitation;
About us
Andover Primary Care Network (PCN) comprises 5 GP
practices and serves a patient population of around 60,000. The Occupational
Therapist will be part of the PCN team headed by the Clinical Director and
consisting of a growing number of PCN staff including clinical pharmacists,
social prescribers, care coordinators and physiotherapists.

The OT
will be part of the Primary Care Network staff team and will work closely with
the Andover Primary Care Mental Health Service.
The OT will receive referrals from the Andover Mental Health Service
Team for patients who benefit from occupational therapy support and an Interest
in mental health is important for this role.
Working independently, the post holder will manage a varied
and complex caseload providing OT assessment and intervention to patients aged
16 and over who are identified as needing occupational therapy. The post holder
will be required to work as part of the multi-disciplinary team offering advice
& recommendations to the team based on the outcome of assessments &
interventions carried out with patients. The post holder will be expected to
work within the limits of their professional competence, skills and knowledge
base.
