Job summary

We are looking for an exceptional nurse to join our team at Mid Hampshire Healthcare which covers the Winchester East rural area.

Main duties of the job

Previous experience of community nursing is a benefit but not essential as we offer a structured training development programme with formal support, provided. If you choose to join us here, many of your new colleagues will have joined us from acute services and found community nursing to be the exciting and challenging next step in their nursing career.

As a community nurse, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

About us

Job responsibilities

1. Main Purpose of role

The main purpose of this role is to provide proactive clinical support to vulnerable patients and those

with complex physical or mental health needs who are considered to be at high risk of future hospital

admission within the Winchester and Andover Primary Care areas. The aim is to improve their quality

of life and healthcare experience and therefore reduce the risk of emergency hospital admission.

The transformation fund was a 12-month project funded by West Hampshire Clinical Commissioning

Group (WHCCG) to:

Reduce re-admissions for patients aged 70 plus.

Increase the use of voluntary sector healthcare support.

Increase the number of and improve the quality of personalised care plans.

Increase the number of patients being discharged to their usual place of residence.

Increase geriatric assessments.

To reduce ambulance conveyances.

Reduction of 999 calls.

Reduce the length of stays for patients aged 70 plus.

Reduction in delayed discharges for patients 70 plus.

To improve the patients quality of life and healthcare experience.

Support carers.
