Job summary

We would like to invite applications from suitably qualified individuals who have the talent, experience and ambition to become our lead for the planning and implementation of an innovative place-based approach to the Ageing Well programme.

The successful applicant will be highly motivated and capable of working independently and autonomously, whilst also communicating with, and working alongside, a broad range of agencies across the spectrum of health and social care.

The successful applicant may have a background in health, social care or the voluntary sector, but will come with a proven record of service design and improvement, and a passion for improving the health and well-being of older people. Excellent communication skills will be essential, as will advanced skills in planning, organisation and leadership.

Main duties of the job

The post-holder will take responsibility for the planning, co-design and implementation of transformative improvement in three key areas of the Ageing Well programme: Enhanced Health in Care Homes, Urgent Crisis Response and Anticipatory Care. Working across general practice, community services, adult social care, mental health and the voluntary sector, the successful applicant will consult with key individuals and agencies to develop a place-based response to the national service specifications for older people.

The Primary Care Networks of the Selby District, along with the Community Services arm of York and Scarborough Teaching Hospitals Foundation Trust (YSTHFT), have partnered with colleagues in local authority and the voluntary sector to agree the joint delivery of expanded and improved services for older people in our locality. Our organisations share a belief that our residents deserve the best care which we can offer them, and a vision for delivering it through a provider alliance.

We therefore seek to appoint an experienced leader who, reflecting our values and beliefs, can help to further our ambitions for this programme of work.

This is an innovative role which is hosted jointly by York and Scarborough Teaching Hospitals NHS Foundation Trust and Selby Town Primary Care Network, but which will have an equally close relationship with colleagues in Adult Social Care and the Tadcaster and Rural Selby Primary Care Network. The role will be based in Selby, working from local authority offices/the local community hospital but closely aligned with community services and PCN staff.

MAIN DUTIES & RESPONSIBILTIES

1. General

· Have, or acquire through training provided by the organisation, the appropriate level of safeguarding and knowledge, skills and practice required for the post and be aware of and comply with the organisation’s safeguarding protection policies and procedures.

2. Programme Management

The post holder will act as programme manager for the Ageing Well Programme and associated developments. They will develop project plans, identify, and track milestones, create action logs, and put in place appropriate governance arrangements. The post holder will develop a risk management framework with an associated risk register detailing controls and mitigations.

The post-holder will, through the programme drive forward the integration of community, primary and social care services in the District of Selby. They will work closely with Primary Care Networks, supporting the development of integrated community/primary care teams and develop relationships with social care providers and the community & voluntary sector organisations working locally.

The post holder will work alongside our community health services provider to address emerging health inequalities and ensure that the needs of vulnerable groups are met. They will pay a lead role realigning community health services so that they support the objectives set out in the NHS Long Term Plan. The main duties and responsibilities of the post holder are set out below.

· Lead the Ageing Well programme

· Work closely with Primary Care and Community Health Services teams to identify opportunities for service and quality improvement

· Lead the development of a community services outcome framework

· Ensure that partner organisations and residents are fully engaged in the programme

· Support the development of Selby Health Matters and other local programmes of work across health and social care

3. Strategic Development

The post holder will provide local strategic overview of primary care and community health services based upon local strategies and national policy. They will progress and deliver service developments. The post holder will incentivise system-wide improvement, placing emphasis on outcomes as well as activity.

The post holder will provide analysis that supports strategy and priority setting, utilising benchmarking and trends. They will support performance intelligence by undertaking analysis of performance, enabling patterns and variations to be identified. The post holder will analyse management, statistical and contractual information for the purpose of strategic planning.

The post holder will develop business plans, identify quality outcomes, and write impact assessments. They will be responsible for making recommendations, providing advice and preparing reports/briefings for senior system leaders.

The post holder will actively undertake patient, public and stakeholder engagement as required to successfully deliver the objectives of this role.

4. Operational Delivery

The post holder will ensure that the ambition for community services and strategic intentions are realised. They will work with local communities on the co-design of services. The post holder will actively lead the redesign of clinically led community care improvement projects.

  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
  • Prince 2 or equivalent relevant experience with a willingness to undertake a formal qualification

Desirable

  • Educated to masters level or equivalent

Experience

  • Experience of leading and implementing key strategies within health, social care or the community and voluntary sector
  • Understanding of the commissioning process and the services necessary to effectively manage and deliver the business and governance responsibilities of a statutory organisation.
  • Understanding of the health and social care environment, including commissioning of health and social care services.
  • Understanding of current and emerging NHS policy, and of policy linked to wider organisations and agendas.
  • A proven track record of communicating and engaging effectively with a wide range of external agencies at all levels.
