Job summary
This is an innovative and exciting role, which is new to NHS organisations, and involves using available data sources to understand the health and well-being profiles of our residents.
A message about recruitment from our clinical director:
"Some years ago, I read a book about the early US space program. It was called "The Right Stuff" and describes how dozens of the very best military pilots were screened for their ability to become astronauts. Although all of them were excellent pilots, only a small number of them made it onto the NASA space program. I mention this because the PCN is looking for astronauts - the very best people- to come with us on our journey of discovery. So, the question is: are you a pilot? Or an astronaut? If you've read this far you might just be an astronaut, or have the ability to train as one, and we would love to hear from you."
Main duties of the job
You will be enthusiastic, meticulous and proactive. Keen to work as part of a team, you will be self-motivated with well-developed communication skills. You will have excellent IT literacy but might not yet have experience of working in health or care settings. Nonetheless, you will be open to learning new skills and ideas, and to developing the care coordination role to its maximum potential. In time you will become an expert resource to the PCN, influencing our digital and data analysis agendas, and making a positive impact on our development.

Selby Town PCN is a group of four General Practices which look after the needs of nearly 50,000 people, in an increasingly collaborative partnership with other organisations such as local authorities, community service providers, our mental health trust and voluntary and community groups.

To engage fully with these workstreams means developing a deep understanding of the needs of our patients and residents. That means we need to get serious about data, and thats where our Digital Care Coordinator comes in.
