Job summary

This is an innovative and exciting role, which is new to NHS organisations, and involves using available data sources to understand the health and well-being profiles of our residents.

A message about recruitment from our clinical director:

"Some years ago, I read a book about the early US space program. It was called "The Right Stuff" and describes how dozens of the very best military pilots were screened for their ability to become astronauts. Although all of them were excellent pilots, only a small number of them made it onto the NASA space program. I mention this because the PCN is looking for astronauts - the very best people- to come with us on our journey of discovery. So, the question is: are you a pilot? Or an astronaut? If you've read this far you might just be an astronaut, or have the ability to train as one, and we would love to hear from you."

Main duties of the job

You will be enthusiastic, meticulous and proactive. Keen to work as part of a team, you will be self-motivated with well-developed communication skills. You will have excellent IT literacy but might not yet have experience of working in health or care settings. Nonetheless, you will be open to learning new skills and ideas, and to developing the care coordination role to its maximum potential. In time you will become an expert resource to the PCN, influencing our digital and data analysis agendas, and making a positive impact on our development.

Selby Town PCN is a group of four General Practices which look after the needs of nearly 50,000 people, in an increasingly collaborative partnership with other organisations such as local authorities, community service providers, our mental health trust and voluntary and community groups.

Job responsibilities

The duties and responsibilities to be undertaken by the Digital Care Coordinator may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the PCN management team, dependent on current and evolving PCN workstreams/priorities:

Identify patients who currently do not use digital channels for healthcare access.

Work with patients to improve digital inclusion.

Work with partner organisations to promote digital inclusion.

Identify and support patients and groups of patients who are digitally excluded.

Maintaining and monitoring the data coding on the clinical system Running system reports and extracting data as required by the PCN for activity reporting. This will utilise both System One & Emis Web clinical systems as well as other available data sources.

Liaising with PCN practices to ensure coding is compliant & consistent.

Implementing data quality assurance on all practice activity data.

Reporting on activity of staff undertaking Additional Roles

Identifying patient cohorts requiring additional medical/social support

Providing data administration support to the PCN management team as required.

Any reasonable requests from the PCN management team.
