Job summary
Selby Town Primary Care Network have an exciting new
opportunity for a motivated individual to join our team of practice based Clinical Pharmacists. Working across 2 practices; Beech Tree Surgery & Escrick Surgery this position will be ideal for an individual who has excellent communication skills & can
work independently as well as within a small team.
The role of a Clinical Pharmacist is to increase capability and
capacity for improving the care of patients within the two practices. The post
holder will act within their professional capacity to support the effective delivery of the GP contract. This may involve
working alone or with other pharmacists currently employed by Beech Tree
Surgery/Escrick Surgery.
Hours: 37.5 per week
Salary: AfC Band 4 (depending on experience)
Main duties of the job
You should be able to organise and prioritise own workload
Ability to communicate both verbally and in writing with all
levels of staff including senior clinicians
Able to function effectively as part of a team and to
provide cross functional cover for all team meetings
Work flexibly and collaboratively with practice staff, patients, care home staff & residents to ensure the appropriate management of medication

Job responsibilities
The duties and responsibilities to be undertaken by the Clinical
Pharmacist may include any or all of the items in the following list. Duties may be varied from time to time under
the direction of the PCN management team/Practice Managers, dependent on
current and evolving PCN workstreams/priorities:
Consult with patients to support medicine optimisation
and delivery.
Review patients who have long term conditions
such as COPD, Asthma, Diabetes etc. to ensure medication is optimal, that
patient compliance is high and where necessary to make recommendations for
change to the GP team.
Undertake routine medication reviews for
patients on repeat prescriptions, change/adapt medication and/or make
recommendations where necessary to the GP team.
Liaise with care homes regarding medication
queries, ensure medication reviews of residents are up to date for existing
& new residents.
Work with care home staff to ensure
compliance of medicines safety in handling and administration.
Attend multi-disciplinary team meetings to
discuss complex patients/palliative care/cancer diagnosis. Use medicines
expertise to support team in making clinical decisions.
Consult with patients via a range of
modalities including face to face and telephone to manage a caseload of common
minor ailments. Within the scope of competency treat, prescribe and signpost
for minor ailments and injury referring where required to GPs and other
healthcare professionals.
Provide telephone support to patients who
have medicine or prescription queries.
Support the prescribing team (GPs and Nurses)
in their use of medication, answer medicine related queries and make
recommendations for change.
Manage patients discharged from hospital;
review and implement changes to medication regimes and liaise with patient and
community pharmacist to ensure robust procedures are in place to ensure
continuity of medicine administration.
Manage MHRA and CAS alerts relating to
medicines safety and make relevant changes throughout the clinic to ensure
compliance and safe operating procedures.
Monitor practice prescribing against RAG list
and make recommendations to the prescribing team. Audit compliance against
current guidelines and provide information and recommendations to the practice
team.
Support practice teams in local and national
campaigns, giving recommendations for effective delivery and maximising
efficacy.
