Job summary

This role is primarily dedicated to delivering high quality physical health checks to our most vulnerable mental health patients. It will involve working closely with our LD and SMI Care Coordinator to ensure checks are delivered in a personalized way.

The role will primarily involve taking responsibility for the delivery of physical health checks to a defined group of patients who are registered with their GP as having a Severe Mental Illness (SMI) or a Learning Disability (LD). Additionally, you will be responsible; working in collaboration with the LD/SMI care coordinator; for producing Personalised care plans and working with system partners to achieve set targets.

Main duties of the job

You will work alongside practice teams in all four PCN practices and develop effective relationships with the local Community Mental Health and Learning Disability teams and Voluntary Sector partners. You will look at delivering health checks in an innovative way that suits the needs of the patient and takes into account and records reasonable adjustments. This may involve delivering the checks alongside practice teams, mental health teams, in other community settings, or in the patients own home.

Selby Town PCN is a group of four General Practices in Selby and Escrick which look after the needs of nearly 50,000 people. Selby PCN has a vision of achieving the best health and well-being for our population.

One of our key aims is improving the health and well-being outcomes for our patients with Severe Mental Illness (SMI) and Learning Disabilities (LD). To achieve this we have three main objectives:

1. Improve uptake and quality of annual physical health checks to patients in these cohorts.

2. Develop novel mechanisms to communicate with and support these patients in the community.

3. Construct effective support network with third parties to provide holistic care approach for patients in these groups.

The duties and responsibilities to be undertaken by the Health Care Assistant are listed below:

Identifying patients to be assessed

Developing appointment slots

Obtaining information of SMI and LD Lists from General Practice

Liaise with mental health teams about patients also under their care.

Offering physical health checks to patients

Undertake SMI physical health checks.

Undertake LD health checks.

Undertake venepuncture.

Administer routine vaccinations.

Carry out ECGs as needed.

Entering data onto electronic patient records

Liaising with the practice nurse or GP where needed

Complete health action plans, help develop personalised care plans.

Deliver opportunistic health promotion and lifestyle advice.

Onward referral when needed e.g., to social prescribing link workers, smoking cessation, exercise referral, drug and alcohol services, community and voluntary sector partners.

Attendance at Mental Health MDT meetings

Participate in training and development.

Develop own knowledge and skills in LD/SMI; become local advocate for LD/SMI to include sharing best practice and staff education.

Visiting patients in home and community settings to carry out checks.

Visit local GP practices to deliver health checks and work with local teams.
