We are looking for an experienced Advanced Nurse Practitioner to join our friendly and professional team in serving our patients.

The successful candidate will be an experienced, motivated and enthusiastic Advanced Nurse Practitioner.

The post is for up to full-time hours.

Main duties of the job

Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and will develop, implement and evaluate programmes/plans of care.

Working as an Advanced Nurse Practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence

Providing direct consultations which dependent upon competency and area of practice, may be via telephone or face to face.

Accountable to: Practice Manager and GP Partners

PURPOSE OF ROLE

Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and will develop, implement and evaluate programmes/plans of care.

Working as an Emergency Care Practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence

Providing direct consultations which dependent upon competency and area of practice, may be via telephone or face to face.

The post holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place. Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential and to prevent unnecessary hospital admission.

The post holder will demonstrate a commitment to providing a quality, evidence based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.

The post holder will support the delivery of policy and procedures, providing clinical leadership to achieve a quality, evidence based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.

The service operates during practice opening hours, which may change depending on local guidelines.

MINIMUM QUALIFICATIONS AND EXPERIENCE REQUIRED FOR THIS POST

  • Recognised professional degree or equivalent
  • Postgraduate study at Masters level or equivalent experience
  • Registered with appropriate professional regulatory body
  • Relevant post registration qualification in autonomous practice or equivalent.
  • Evidence of relevant post registration experience
  • Independent Prescribing or evidence of working to PGDs with advanced knowledge (AHPs)
  • Evidence of continuing professional development
  • Excellent written and verbal communication skills
  • Computer literacy
  • Knowledge of primary and urgent care issues
