Advanced Nurse Practitioner Burnham and Berrow Medical Centre
We are looking for an advanced nurse practitioner to join our friendly and supportive Somerset practice and expand our multi-disciplinary team. The post is for up to 37.5 hours per week (part time hours will be considered). Salary will be dependent on experience with additional remuneration for those that are prescribing, fully competent Nurse Practitioners with primary care experience and mentoring skills.
You will be an enthusiastic, caring and motivated professional with excellent clinical, organisational and communication skills, who has the ability to work autonomously but also be a key member of the growing multidisciplinary team at the practice
Main duties of the job
As an experienced ANP, you will play a key
role in the success of the Practice and will be responsible for triaging,
assessing, diagnosing, treating and discharging patients autonomously as well
as supporting improving service provision.
You will be able to demonstrate the ability
to make differential diagnosis using decision making and problem solving skills
whilst making professional autonomous decisions for which you are accountable.
The successful candidate will possess the ability to work both independently
and to work collaboratively as part of a fantastic multi-disciplinary team with
other health care professionals and disciplines. NMC requirements for advanced
practice must be met.

Burnham and Berrow Medical Centre is a
large, well established practice with an on-site pharmacy, which provides a
wide range of services to around 15,000 patients. We are a friendly practice in

The post holder is an
experienced nurse who, acting within their professional boundaries, will
provide care for the presenting patient from initial history taking, clinical

KEY RESPONSIBILITIES
Clinical
practice
- Assess,
diagnose, plan, implement and evaluate treatment/interventions and care
for patients presenting with an undifferentiated diagnosis
- Clinically
examine and assess patient needs from a physiological and psychological
perspective, and plan clinical care accordingly
- Assess,
diagnosis, plan, implement and evaluate interventions/treatments for
patients with highly complex needs
- Proactively
identify, diagnose and manage treatment plans for patients at risk of
developing a long-term condition (as appropriate)

and manage both acute and chronic conditions, integrating both drug- and
non-drug-based treatment methods into a management plan

and review medication for therapeutic effectiveness, appropriate to
patient needs and in accordance with evidence-based practice and national
