Main duties of the job
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. Friendly, supportive and innovative team environment.
We are looking for a
salaried GP who wants to work with an ethnically and economically diverse

About us
We are proud to excel in our clinical outcomes and are very
proud of working in City and Hackney which has a history of valuing and
investing in primary care. We are a training practice for undergraduate
students, GPs and practice nurse trainees. The local confederation has a
fantastic salaried doctor network with regular meetings.
We are a large but close team of the usual staff but also
innovative PCN roles such as a vocational OT, mental health worker and
physician associate as well as a fantastic team of clinical pharmacists and
FCPs and a dynamic wellbeing team. We are a 7 partner practice one of which is
our lead nurse and another is our experienced ANP. We have 11 salaried doctors,
many who have stayed after training with us. We pride ourselves on our
supportive and nurturing environment. All salaried doctors have a mentor
session with a partner every month. We have daily case discussion meetings as
well as the usual MDT meetings and a daily morning huddle. There is always a
senior supervising doctor to support you and we welcome a knock on the door to
review a patient with you.
We are about to move intoan amazing new building
incorporating a very special building (Google Portico Hackney) so there is a
lot of excitement in the practice.

Clinical Responsibilities
In accordance with the Practice timetable, as agreed,
the post-holder will make him/her-self available to undertake a variety of
duties including surgery consultations, telephone consultations and queries,
visiting patients at home, checking and signing repeat prescriptions and
dealing with queries, paperwork, referrals and correspondence in a timely
manner.
Making professional, autonomous decisions in relation
to presenting problems, whether self-referred or referred from other health
care workers within the Practice.
Assessing the health care needs of patients with
undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early
signs of illness.
In consultation with patients and in line with current
Practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the
caseload and referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes
to agreed standards.
Collecting data for audit purposes.
Compiling and issuing computer-generated acute and
repeat prescriptions (avoiding hand-written prescriptions whenever possible).
Prescribing in accordance with the Practice
prescribing formulary (or generically) whenever this is clinically appropriate.
Ensuring that all work carried out which carries a
private fee is properly recouped and accounted for; e.g. Medical Reports,
Insurance reports, Private letters to third Parties etc.
